One risk factor for anastomotic leakage (AL) after esophagectomy with retrosternal gastric reconstruction is excessive compression of the gastric tube at the thoracic inlet. In this study, we evaluated the effect of our modified procedure to reduce AL by placing the esophagogastric anastomosis below the thoracic inlet. Between January 2008 and December 2022, 174 consecutive patients underwent subtotal esophagectomy with retrosternal gastric pull up, followed by circular stapler anastomosis in our hospitals.

Article Synopsis
  • Cisplatin causes damage to renal proximal tubular cells through mitochondrial impairment, leading to cell death and inflammation.
  • Dichloroacetate (DCA) protects against this damage by enhancing energy production and influencing specific apoptotic pathways, particularly by inhibiting JNK and caspase pathways, and increasing cFLIP levels.
  • In murine models, DCA reduced kidney injuries associated with cisplatin treatment, suggesting its potential therapeutic role against nephrotoxicity while promoting some ROS production and inflammation without full impairment.

Article Synopsis
  • - Cutaneous apocrine carcinoma (CAC) is a rare skin cancer originating in apocrine sweat glands, with no established evidence-based treatments available.
  • - A 61-year-old female diagnosed with axillary CAC and lymph node metastasis had surgery, revealing multiple metastases, and her tumor tested positive for estrogen and progesterone receptors, as well as HER2.
  • - The patient received chemotherapy, including anti-HER2 drugs, and hormone therapy, resulting in no recurrence for over three years, suggesting that breast cancer treatments could be effective for CAC due to similarities between apocrine glands and mammary glands.

Article Synopsis
  • The study investigated how the CHADS-VASc score and prior oral anticoagulant (OAC) use affect endovascular treatment (EVT) outcomes in acute ischemic stroke patients with atrial fibrillation (AF).
  • Out of 6984 patients analyzed, a significant portion had high CHADS-VASc scores, which correlated with increased risks of complications and decreased independence post-treatment; however, prior use of direct oral anticoagulants (DOACs) improved some outcomes for high-risk patients.
  • Findings suggest that using DOACs in high- and selected moderate-risk AF patients prior to EVT can enhance treatment success, indicating potential benefits for preventive DOAC therapies.

Peroxisome proliferator-activated receptor-alpha (PPAR-α) and its exogenous activators (fibrates) promote autophagy. However, whether the deleterious effects of PPAR-α deficiency on doxorubicin (DOX)-induced podocytopathy are associated with reduced autophagy remains to be clarified. We investigated the mechanisms of PPAR-α in DOX-induced podocytopathy and tubular injury in PPAR-α knockout (PAKO) mice and in a murine podocyte cell line.
