Objectives: The efficacy and safety of a sedation regimen combining dexmedetomidine and midazolam during endoscopic submucosal dissection for upper gastrointestinal tumors remains unclear. In this study, we aimed to evaluate the efficacy and safety of this sedation regimen, where non-anesthesiologists performed sedation.

Methods: Sixty-eight patients who underwent endoscopic submucosal dissection for upper gastrointestinal tumors, sedated by non-anesthesiologists, were retrospectively evaluated.

The multidisciplinary team (MDT) approach has become the standard for perioperative patient care. At our institution, a multidisciplinary perioperative care team called "Surgical, Nutrition and Rehabilitation Integrated Services for Excellence Team (SUNRISE)" was established for all patients with gastrointestinal cancer undergoing surgery. This study aimed to elucidate the significance of SUNRISE as a perioperative MDT by comparing short-term postoperative outcomes before and after the introduction of SUNRISE in patients with colorectal cancer.

Article Synopsis
  • * Liquid biopsy has the potential for detecting these mutations, but current methods struggle with sensitivity and accuracy.
  • * The study explored oligoribonucleotide interference-PCR (ORNi-PCR) coupled with real-time PCR or droplet digital PCR (ddPCR), finding that ORNi-PCR methods are more effective at detecting mutant DNA for early diagnosis and monitoring of recurrent PDAC.

Article Synopsis
  • - Edoxaban, a factor Xa inhibitor, is usually metabolized into an active metabolite called M-4, which has minimal antithrombotic effects in healthy individuals; however, certain conditions can elevate its plasma concentration significantly.
  • - A 68-year-old man with pancreatic cancer was prescribed edoxaban along with rifampin and clarithromycin for lung disease but developed acute kidney injury after surgery, leading to unusually high levels of M-4 in his system.
  • - This case highlights the potential for increased M-4 levels in patients with kidney issues who are taking edoxaban alongside drugs that affect its metabolism, suggesting that these combinations could be more clinically impactful than previously understood.

Purpose This study aimed to elucidate the relationship between early-stage weight loss (WL) during hospitalization after pancreatic surgery and prognosis and investigate risk factors affecting WL. Methods We included 68 patients diagnosed with pancreatic ductal adenocarcinoma (PDAC) who underwent radical surgery. The %WL value was calculated based on the percentage of body weight at discharge compared with the body weight at admission.
