We describe two patients who developed gluteal compartment syndrome after total knee arthroplasty (TKA) carried out under epidural analgesic infusion and light sedation. To our knowledge, this occurrence has not been described previously after TKA.

In brief: Forty-eight trained runners with runner's knee were examined, treated, and followed for eight months to identify the causes of this injury and its response to a particular regimen of conservative management. The subjects (41 men, 7 women), ranged in age from 20 to 40. Most trained from 40 to 80 km per week and had been running for four years or less; half had recently altered their training routine, the most common change being an increase in distance.

The results of an 18-month prospective clinical trial of external fixation in the management of open tibial fractures at Groote Schuur Hospital, Cape Town, are reported. Liaison with the Department of Mechanical Engineering at the University of Cape Town has resulted in the development of significant improvements on the methods previously described. The value of external fixation in the management of tibial fractures is confirmed; problems encountered with this particular method are described.
