Gender-affirming mastectomy surgery is highly desired within both transmasculine and nonbinary patient populations. The development of cardiac arrhythmias has been reported within this population. Acute intraoperative bradycardia in patients undergoing gender-affirming mastectomy has not been well described previously.

Skin grafting is a fundamental tool in plastic surgery for the resurfacing of wounds resulting from burns, necrotizing infections, trauma, oncologic resections, donor site defects, and other causes. Compared with split-thickness skin grafts, full-thickness skin grafts (FTSGs) undergo less secondary contracture and often result in better aesthetic outcomes. To assure graft take, FTSGs require thorough defatting and adequate contact between the graft and the underlying wound bed.

Drain placement is commonplace after many plastic surgery procedures to evacuate excess blood and fluid. Tranexamic acid (TXA) is an antifibrinolytic that has been shown to decrease bleeding and fluid production at surgical sites and can be administered orally, intravenously, and topically. We sought to evaluate the efficacy of our ERP including use of liposomal bupivacaine [Exparel] in these cases, assess the efficacy of TXA in reducing postoperative complications, and compare need for revisionary surgery between periareolar and DI mastectomy techniques.
