European S2k guidelines for hidradenitis suppurativa/acne inversa part 2: Treatment.

Introduction: This second part of the S2k guidelines is an update of the 2015 S1 European guidelines.

Objective: These guidelines aim to provide an accepted decision aid for the selection, implementation and assessment of appropriate and sufficient therapy for patients with hidradenitis suppurativa/acne inversa (HS).

Methods: The chapters have been selected after a Delphi procedure among the experts/authors.

Toward an interoperable, intraoperative situation recognition system via process modeling, execution, and control using the standards BPMN and CMMN.

Purpose: For the modeling, execution, and control of complex, non-standardized intraoperative processes, a modeling language is needed that reflects the variability of interventions. As the established Business Process Model and Notation (BPMN) reaches its limits in terms of flexibility, the Case Management Model and Notation (CMMN) was considered as it addresses weakly structured processes.

Methods: To analyze the suitability of the modeling languages, BPMN and CMMN models of a Robot-Assisted Minimally Invasive Esophagectomy and Cochlea Implantation were derived and integrated into a situation recognition workflow.

With over 400,000 data points and 2400 languages, Grambank is the largest comparative grammatical database available.

[Acne inversa: A rare disease with far-reaching consequences for those affected : Experiences and wishes of patients].

Patients affected by acne inversa (hidradenitis suppurativa) initially notice skin changes in atypical areas of the skin. These changes can include purulent inflammation, painfully filled and red-discolored bumps or oozing and strongly smelling, small to larger openings. In order to be able to receive medical help, patients must present to the general practitioner, report their complaints and also show the lesions.

Background: Emerging drug-resistant strains of Candida albicans have led to the recurrence of fungal disease, rendering conventional drug therapies ineffective. Background: Practising medicine at an expert level requires skills beyond medical expert knowledge. Research shows that newly appointed consultants feel less prepared to deal with leadership issues compared to issues regarding medical expertice. Newly graduated (NG) doctors and residents in particular struggle with leadership and organisational issues.

Implementing pharmacogenomics decision support across seven European countries: The Ubiquitous Pharmacogenomics (U-PGx) project.

Clinical pharmacogenomics (PGx) has the potential to make pharmacotherapy safer and more effective by utilizing genetic patient data for drug dosing and selection. However, widespread adoption of PGx depends on its successful integration into routine clinical care through clinical decision support tools, which is often hampered by insufficient or fragmented infrastructures. This paper describes the setup and implementation of a unique multimodal, multilingual clinical decision support intervention consisting of digital, paper-, and mobile-based tools that are deployed across implementation sites in seven European countries participating in the Ubiquitous PGx (U-PGx) project.

Professional identity formation in the transition from medical school to working life: a qualitative study of group-coaching courses for junior doctors.

Background: The transition from student to medical doctor is challenging and stressful to many junior doctors. To practice with confidence and professionalism the junior doctors have to develop a strong professional identity. Various suggestions on how to facilitate formation of professional identity have been offered including the possible positive effect of group-coaching courses.

Influence of catch-up growth on abdominal fat distribution in very low birth weight children - cohort study.

Background/aims: A rapid catch-up growth in very low birth weight has been associated both with a higher height growth and a higher risk to metabolic disturbances, including insulin resistance and its consequences. Abdominal fat distribution in early postnatal life may play a role in these outcomes and can help in addressing this neonatal dilemma. This study aimed to compare abdominal fat distribution among very low birth weight (VLBW) children with and without rapid catch-up growth.

[A group coaching course supports professional development of doctors under specialist training].

A group coaching course for doctors under specialist training was evaluated. The course consisted of group coaching, theoretical presentations and practical exercises over a three-month period. The confidential forum of colleagues was of utmost importance, creating recognition and social support in contrast to the collegiate competitiveness of the workplace.

Enterovirus 71 outbreak in Northern Sydney, 2013: case series and initial response.

Aim: Enteroviruses are a common cause of childhood disease which may manifest in a variety of ways. Enterovirus 71 (EV71) is a subtype of enterovirus which can cause meningoencephalomyelitis resulting in neurological sequelae including lethargy, weakness, ataxia, sleep myoclonus, urinary retention and, in severe cases, cardiorespiratory collapse due to neurogenic pulmonary oedema. EV71 was responsible for outbreaks in South East Asia in 1997-1998, in Western Australia in 1999 and in Sydney in 2000-2001.

Text data extraction for a prospective, research-focused data mart: implementation and validation.

Background: Translational research typically requires data abstracted from medical records as well as data collected specifically for research. Unfortunately, many data within electronic health records are represented as text that is not amenable to aggregation for analyses. Knowledge sharing among healthcare infection preventionists: the impact of public health professionals in a rural state.

Background: Healthcare-associated infections are a major source of morbidity and mortality in the United States. Infection Preventionists (IPs) are healthcare workers tasked at overseeing the prevention and control of these infections, but they may have difficulties obtaining up-to-date information, primarily in rural states. The objective of this study was to evaluate the importance of public health involvement on the knowledge-sharing network of IPs in a rural state.

Telemedicine and pediatric radiology: a new environment for training, learning, and interactive discussions.

Objective: To report the experience of the Brazilian Program of Pediatric Teleradiology in combining teleconferencing and a virtual learning environment for services integration, collaborative research, and continuing education in pediatric radiology.

Materials And Methods: We performed virtual meetings from March 2005 to October 2010 on pediatric radiology-related themes, using a combination of videoconferences and Web conferences, which were recorded and made available in an open-source software (Moodle) for reuse.

Results: We performed 58 virtual sessions: 29 anatomical-clinical-radiological sessions, 28 on upgrading themes, and 1 virtual symposium.

Reduction of the complication rate in Liechtenstein hernia repair.

Background: Inguinal hernia is one of the most common pathologies in the surgical setting. The introduction of the Lichtenstein technique in 1989 (tension-free hernioplasty with polypropylene mesh) represented one of the most significant breakthroughs in the treatment of this condition since Bassini's hernia repair. The aim of this study was to know the most significant predictive variables of complications in Lichtenstein hernioplasty and if some changes introduced in the technique could reduce these complications.

A highly specific algorithm for identifying asthma cases and controls for genome-wide association studies.

Our aim was to identify asthmatic patients as cases, and healthy patients as controls, for genome-wide association studies (GWAS), using readily available data from electronic medical records. For GWAS, high specificity is required to accurately identify genotype-phenotype correlations. We developed two algorithms using a combination of diagnoses, medications, and smoking history.

dictyBase (http://dictybase.org) is the model organism database for Dictyostelium discoideum. It houses the complete genome sequence, ESTs and the entire body of literature relevant to Dictyostelium.

Dorsolateral approach for arthrocentesis of the centrodistal joint in horses.

Objective: To develop a dorsolateral approach to the centrodistal (distal intertarsal) joint in horses and compare its success rate with that of the traditional medial approach in that joint.

Sample Population: 25 cadaveric equine hind limbs, ultrasonographic images, and radiographic views of the tarsal region of 5 and 59 healthy horses, respectively, and 22 horses with a clinical indication for centrodistal joint centesis.

Procedures: The dorsolateral approach was established anatomically (3 cadaveric limbs), ultrasonographically (5 horses), and radiographically (59 horses).

Notification - what's it all about?

Background: General practitioners are often unsure of their obligation to notify public health units of infectious disease in their patients. A measles case study in which contacts missed out on appropriate prophylaxis due to failure to notify by a GP and hospital staff is discussed.
