This case report presents a 64-year-old male diagnosed with sinonasal primary squamous cell carcinoma (SNSCC), a rare and aggressive upper aerodigestive tract malignancy. Initially, he presented with unilateral recurrent epistaxis. Imaging and histopathology confirmed the diagnosis.

- Initial symptoms include a sore throat and difficulty swallowing, which can quickly worsen to drooling, intense pain when swallowing, and a hoarse voice.
- Prompt diagnosis and treatment, including airway management and intravenous antibiotics, are critical to prevent life-threatening complications.
Transmission electron microscopy using resin sections still remains an exceptionally useful tool in evaluating cellular ultrastucture within tissue. For the endothelium the best method for maintaining such structure is perfusion fixation: fixing the tissue under physiological pressure. Here the focus is on a method of maintaining the vascular wall structure including the endothelial glycocalyx and extending this with tilt series tomography.
