Case: We herein present a case involving a 23-year-old man with an open fracture of the distal portion of the left femoral diaphysis, substantial bone loss, and soft tissue injuries. He was initially treated with antibiotics, limb stabilization, and surgical debridement. Smart Agent is a web-based solution for establishing bidirectional communication between an infusion pump and an electronic health record (EHR). It eliminates the need for clinician double check of medication administration using an infusion pump. Because the clinician already is using the EHR to review patient health information and update status, the addition of the web service would help eliminate the potential for human error when using a manual system.

The conductive and diffusional behavior of electrolytes in media with different dielectric and viscoelastic properties is investigated. A revised model to separate the contribution of dissociated and nondissociated species to the diffusion coefficients determined with NMR is proposed. Impedance spectroscopy is used to measure the ionic conductivity of lithium salts in aqueous medium, ionic liquids in aprotic solvents, and hydrogel polyelectrolytes.

In the study of the electric properties of electrolytes, the determination of the diffusion coefficients of the species that intervene in the charge transport process is of great importance, particularly that of the free ions (D(+) and D(-)), the only species that contribute to the conductivity. In this work we propose a model that allows, with reasonable assumptions, determination of D(+) and D(-), and the degree of dissociation of the salt, α, at different concentrations, using the diffusion coefficients experimentally obtained with NMR. Also, it is shown that the NMR data suffice to estimate the conductivity of the electrolytes.

In liquids and in polymeric membranes, a precise determination of their transport properties is of paramount importance. In this work, an NMR method to measure sequentially the solubility and diffusion coefficients of carbon dioxide in liquids (n-alkanes and 1-alkanols) and in polymer membranes (polyethylene, polybutadiene, and polycarbonate) is described. The results show that NMR measurements are very reproducible and in good agreement with those determined by other methods.

Surface modification reactions on needle-like sepiolite using alkyl and functional silanes have been carried out in the form of aqueous gels. In contrast with modifications in organic solvents, reactions in water make it possible to modify the surface of almost-individual sepiolite fibers and produce either a continuous coating or a nanotexturization of the sepiolite fiber surface, depending on the reaction conditions. This clean procedure substitutes advantageously organic solvent surface modifications and allows the tuning of surface properties such as specific surface area, wetting behavior, and chemical functionalization.

The scope of this work is the comparative analysis in terms of grafting rate, structure of the grafted layer, and wetting behavior of three series of silica nanoparticles modified with alkyltrimethoxysilanes by using conventional heating with and without acid catalysis, and microwave irradiation. A comprehensive characterization of the grafted layer by means of Fourier transform infrared (FTIR), microanalysis, and solid state NMR techniques has shown that microwave irradiation provokes a pronounced increase in the loading rate compared to conventional heating. This microwave effect is outstanding in the case of the reactions with methyltrimethoxysilane, because of the acceleration of the condensation rate.

The aim of this study is to discuss the meanings associated with the CD4 lymphocyte count and HIV plasma viral load (VL) for patients living with AIDS and the attending physicians, seeking to analyze the impacts of the increasing use of these tests in the treatment setting. A qualitative study was performed in two HIV/AIDS referral centers with participant observation and semi-structured interviews with 27 patients living with AIDS and four physicians. Observation of the medical consultations showed that they are quick, objective, and centered on the CD4 and VL test results, thus reinforcing a hegemonic view of medical knowledge and a biomedical perspective that instrumentalizes their practice.

A comparative study of the dielectric relaxation behavior of two structurally close polymers containing aliphatic-aromatic side groups was carried out in order to get a better understanding on how slight differences in chemical structure affect the molecular responses to perturbation fields. Specifically, chain dynamics of poly(2-acryloyloxyethyl-2-naphthalene-2-ylacetate) and poly(2-methacryloyloxyethyl-2-naphthalene-2-ylacetate) were studied by broadband dielectric spectroscopy in the frequency range of 10(-2)-10(8) Hz and temperature window of 298-403 K. Also, the relaxation behavior of (2-acetyloxyethyl-2-naphthalene-2-ylacetate), model compound of the polymer side groups, was analyzed.

The modification of Aerosil 200 has been carried out using methoxysilanes in toluene reflux, with p-toluenesulfonic acid as the catalyst. Both trimethoxyalkyl silanes (methyl, ethyl, propyl, butyl, hexyl, octyl, and octadecyl) and trialkylmethoxy silanes (trimethyl and dimethyloctyl) have been used. The surface has been studied by 29Si NMR, 13C NMR, elemental analysis, thermogravimetry, water contact angle, and BET analysis.

In April/2004, a group of people traveled to a farm in a town in the south of Bahia and afterwards, three teenagers from the same family developed symptoms of severe septicemia and two died. The Health Department carried out an epidemiologic, clinical, laboratory and environmental investigation that resulted in the identification of Chromobacterium violaceum, in biological material from one of them, water and soil. This is the first report of the disease in Bahia State.
