Penile prosthesis (PP) implantation is feasible as an outpatient surgery. The present study describes the surgical process and establishes a consensus for improving the care circuit for outpatient PP implantation in Spain. Background: While various treatment modalities exist, penile prosthesis (PP) implantation offers a permanent solution for patients that do not respond to or are not suitable for other treatment options for erectile dysfunction.

Aim: The aim of this study was to establish stretched penile length before surgery as a reliable determining factor of penile length after penile implant surgery.

Methods: For this study, data on penile length before and after PP surgery from three European centers was analyzed.

Article Synopsis
  • This study tested an ultra-low-dose estriol vaginal gel (0.005%) to see if it could prevent urinary tract infections (UTIs) in postmenopausal women with genitourinary syndrome of menopause.
  • The clinical trial involved 108 women who were divided into two groups: one received the estriol gel, while the other got a placebo gel, and they were monitored for 24 weeks.
  • Results showed a 26% lower rate of UTIs in the group using estriol compared to placebo, indicating the gel is both safe and effective in reducing UTI occurrences and improving vaginal health.

Introduction: The aim of this study is to compare outcomes of SRP (salvage radical prostatectomy) with SCAP (salvage cryoablation of the prostate) in local radio-recurrent PCa (prostate cancer) patients.

Materials And Methods: A retrospective analysis of a multicentric European Society of Uro-technology (ESUT) database was performed. Data on patients with local recurrent PCa after radiotherapy who underwent salvage treatment were collected.

Purpose: To assess the clinical performance of ProsTAV, a blood-based test based on telomere associate variables (TAV) measurement, to support biopsy decision-making when diagnosing suspicious prostate cancer (PCa).

Methods: Preliminary data of a prospective observational pragmatic study of patients with prostate-specific antigen (PSA) levels 3-10 ng/ml and suspicious PCa. Results were combined with other clinical data, and all patients underwent prostate biopsies according to each center's routine clinical practice, while magnetic resonance imaging (MRI) before the prostate biopsy was optional.

Article Synopsis
  • Penile prosthesis (PP) is a key treatment for Peyronie's disease (PD) in patients with refractory erectile dysfunction (ED), but this study evaluates its use in PD patients without ED.
  • A total of 636 patients had PP for PD from 2012 to 2022, with 168 included in the study, showing that the mean age was 56.3 years and curvature angles were similar between those with and without ED.
  • Results indicated high satisfaction rates (around 88%) for both groups post-surgery, and fewer mechanical failures in the no-ED group; thus, PP may be a safe and effective option for PD treatment, but more research is needed.

The objective of this study was to evaluate and compare efficacy and safety of two different Disposable circumcision suture devices (DCSDs). A prospective comparative non-randomized multicenter study was performed between November 2019 and February 2023. Patients underwent circumcision using a DCSD (CircCurer or the ZSR device) according to the surgeon preference and device availability.

Erectile dysfunction (ED) is a frequent and difficult-to-treat condition in diabetic men. Protein kinase C (PKC) is involved in diabetes-related vascular and cavernosal alterations. We aimed to evaluate the role of PKC in endothelial dysfunction and NO/cGMP impairment associated with diabetic ED in the human corpus cavernosum (CC) and penile resistance arteries (PRAs) and the potential mechanisms involved.

Article Synopsis
  • Surgery is often chosen for Peyronie's disease (PD) due to its potential effectiveness despite risks and variable outcomes; the study investigates factors affecting patients' willingness to undergo such surgery.
  • Conducted in five European centers from 2016-2020, the study analyzed data from 343 patients regarding their age, treatment history, and severity of PD symptoms to determine their attitude toward surgery.
  • Results showed that about 37% of patients were willing to consider surgery, with factors like younger age influencing this willingness, despite many experiencing significant issues such as erectile dysfunction and penetration difficulties.

Context: Recommendations regarding the management of penile size abnormalities and dysmorphophobia are important in guiding evidence-based clinical practice.

Objective: To present a summary of the 2023 European Association of Urology sexual and reproductive health evidence-based recommendations for the management of penile size abnormalities and dysmorphophobia.

Evidence Acquisition: A broad and comprehensive scoping exercise covering all areas of the guidelines was performed.

: The relationship between male infertility (MI) and testicular cancer (TC) is bilateral. On one hand, it is well-established that patients diagnosed with TC have a high risk of pre- and post-treatment infertility. On the other hand, the risk of developing TC in male infertile patients is not clearly defined.

Article Synopsis
  • Current pathways for diagnosing prostate cancer can result in unnecessary biopsies; researchers developed a new model called ProsTAV® using telomere analysis to improve diagnosis.
  • In a study with 1,043 patients, ProsTAV® showed good predictive ability, with a ROC curve area of 0.71, and could potentially avoid 33% of biopsy procedures.
  • The model's sensitivity was high at 90%, while specificity was lower at 33%, indicating it is better at confirming significant prostate cancer than ruling it out.

To review the evidence of clinical efficacy of low-intensity extracorporeal shock wave therapy (Li-ESWT) for the treatment of erectile dysfunction (ED). A search on PubMed using Medical Subject Headings terms [((low intensity extracorporeal shockwave therapy) OR (Li-ESWT)) AND (erectile dysfunction)] was conducted in August 2022, to obtain studies on the use of Li-ESWT for the treatment of ED. Its success rate in terms of International Index of Erectile Function-5 (IIEF-5) score and Erection Hardness Score (EHS) improvement was recorded and analysed.

Background: Stromal interaction molecule (STIM)/Orai calcium entry system appears to have a role in erectile dysfunction (ED) pathophysiology but its specific contribution to diabetic ED was not elucidated.

Aim: To evaluate STIM/Orai inhibition on functional alterations associated with diabetic ED in rat and human penile tissues and on in vivo erectile responses in diabetic rats.

Methods: Rat corpus cavernosum (RCC) strips from nondiabetic (No DM) and streptozotocin-induced diabetic (DM) rats and human penile resistance arteries (HPRA) and corpus cavernosum (HCC) from ED patients undergoing penile prosthesis insertion were functionally evaluated in organ chambers and wire myographs.

Conservative and medical treatments are considered the first step in ischemic priapism (IP) management, although there is no clear evidence regarding their efficacy. We conducted a systematic review on behalf of the EAU Guidelines panel on Sexual and Reproductive health to analyse the available evidence on the efficacy and safety of conservative and medical treatment for non-sickle cell disease-related IP. Databases searched for relevant literature investigating efficacy and safety of conservative measures and medical treatment for IP included Medline, EMBASE, Cochrane Libraries and clinicaltrial.

Targeted therapy (TT) for prostate cancer (PCa) aims to ablate the malignant lesion with an adequate margin of safety in order to obtain similar oncological outcomes, but with less toxicity than radical treatments. The main aim of this study was to evaluate the recurrence rate (RR) in patients with primary localized PCa undergoing mpMRI/US fusion targeted cryotherapy (FTC). A secondary objective was to evaluate prostate-specific antigen (PSA) as a predictor of recurrences.

The Peyronie's Disease Questionnaire (PDQ) is a 15-question self-reported questionnaire that evaluates the severity and physical and psychosexual issues of Peyronie's disease (PD) symptoms in three scales: "psychological and physical symptoms," "penile pain," and "symptom bother." Previous studies validated the PDQ US version and confirmed its test-retest reliability and responsiveness. The aim is to translate and validate the Spanish version of the PDQ to be used in the clinical practice and in PD research studies in Spain.
