Endovascular aneurysm repair requires a suitable anatomy for placement of covered stent grafts, which includes a non-aneurysmal distal landing zone within the common iliac arteries. Patients with iliac artery aneurysms, thus, are often not candidates for this minimally invasive repair, as extension of the graft to an appropriate site would cover the internal iliac artery and result in significant symptoms related to poor pelvic perfusion. We present two cases of common iliac artery aneurysms treated by modified endovascular techniques with good results.

Article Synopsis
  • Patients on anticoagulants often undergo urgent vascular procedures, raising concerns about increased bleeding due to high prothrombin time/international normalized ratio (INR).
  • A study analyzed 1,333 patients to assess safety in those with an INR > 1.5 during arterial procedures, comparing them to a control group with lower INR levels.
  • While the study group showed more anticoagulant use and had slightly more bleeding complications, the overall complication rates were low, suggesting that modern techniques like ultrasound guidance and closure devices enable safe endovascular procedures even in patients with elevated INR.
