Objective: Efficacy of thoracic endovascular aortic repair (TEVAR) for chronic type B aortic dissection (CTBAD) is dependent on eliminating retrograde false lumen perfusion and remodeling the aorta. We describe the efficacy of a novel transcatheter electrosurgical technique to fenestrate the dissection flap and create a distal seal zone for TEVAR in CTBAD.

Methods: A retrospective review of the Emory Aortic Database from 2016 to 2023 identified 33 patients who underwent TEVAR with intentional endovascular rupture of the dissection flap (Knickerbocker; KNICK) for CTBAD.

Introduction: Family medicine clerkships utilize a broad set of objectives. The scope of these objectives cannot be measured by one assessment alone. Using multiple assessments aimed at measuring different objectives may provide more holistic evaluation of students.

Ethanol is metabolized by alcohol dehydrogenase to acetaldehyde and induces cytochrome P450 2E1 (CYP2E1), which generates reactive oxygen species that cause inflammatory liver damage. Clomethiazole, a drug approved for alcohol withdrawal treatment (AWT) in some European countries, inhibits CYP2E1. We hypothesized that clomethiazole would lead to a faster reduction in oxidative stress, inflammatory cytokines, and liver enzymes compared to diazepam treatment.

Article Synopsis
  • Thoracic endovascular aortic repair (TEVAR) is an effective treatment for complicated acute type B aortic dissection (aTBAD), but questions remain regarding coverage length and its effects on aortic health.
  • A study reviewed 92 patients who underwent TEVAR, comparing standard coverage (zones 3 and 4) to extended coverage (zones 3 to 5), revealing extended TEVAR required longer coverage and led to better outcomes in terms of false lumen closure and fewer reinterventions.
  • Overall, extended TEVAR was found to be safe and reduced complications, but more long-term follow-up is needed to confirm any potential survival advantages over standard procedures.

Background: The pathophysiology and behavior of acute type B intramural hematoma (TBIMH) is poorly understood. The purpose of this study is to characterize the pathophysiology, fate, and outcomes of TBIMH in the endovascular era.

Methods: A retrospective analysis of a US Aortic Database identified 70 patients with TBIMH from 2008 to 2022.

Background: African American women are disproportionately at risk for HIV infection. To increase women's readiness to consider taking pre-exposure prophylaxis (PrEP), we conducted a pilot study of Women Prepping for PrEP Plus (WP3+). Adapted from an evidence-based HIV risk reduction intervention for African American couples who are HIV-serodiscordant, WP3+ is a group-based culturally congruent program designed for African American women without HIV.

Importance: Health care algorithms are used for diagnosis, treatment, prognosis, risk stratification, and allocation of resources. Bias in the development and use of algorithms can lead to worse outcomes for racial and ethnic minoritized groups and other historically marginalized populations such as individuals with lower income.

Objective: To provide a conceptual framework and guiding principles for mitigating and preventing bias in health care algorithms to promote health and health care equity.

Article Synopsis
  • The study compared PCSK9 levels in individuals with co-existing HIV and hepatitis C (HIV/HCV) vs. those with HIV alone, finding that PCSK9 levels were higher in the HIV/HCV group but not significantly so (P = 0.06).
  • Following direct-acting antiviral (DAA) therapy for HCV, PCSK9 levels significantly declined in the HIV/HCV group, reaching levels similar to the HIV alone group (P = 0.003 and P = 0.02).
  • The drop in PCSK9 was associated with reductions in inflammatory markers, suggesting that elevated PCSK9 in HIV/HCV may be more related to inflammation rather than cholesterol levels.

With reference to a poem "Dialogue between two lovers" and a fable "The Last Love", essential aspects of love are described: love as access to the world, as a path to wisdom, self-knowledge and virtue, love as fear of loss and the desire to merge, love as encounter in dialogue, love as care, love as ecstasy, a feeling of pleasure and happiness. These are based on three theories of love from the beginnings of philosophy in ancient Greece by Plato and Aristotle. In the article they are considered from a philosophical-psychiatric perspective and supplemented by newer interpretations.

The essential philosophical theories about love are contained in a poem and a fable. Although there are many connections between love, psychological experience and psychiatric disorders, love seems to be suspect in psychiatry. Possibly it is a basic complex of psychiatry not only to withdraw from love, but also to exclude the fundamental issues of sexuality and parenting of the mentally ill.

Use of the American Society of Anesthesiologists (ASA) physical status classification is important for periprocedural risk stratification. However, the collective effect after adjustment for the Society for Vascular Surgery (SVS) medical comorbidity grading system on long-term all-cause mortality, complications, and discharge disposition is unknown. We examined these associations in patients after thoracic endograft placement.

Agents that act at the N-methyl-D-aspartate receptor (NMDAR), such as ketamine, have gained increasing attention as rapid-acting antidepressants; however, their use has been limited by potential neurotoxicity. Recent FDA guidance requires a demonstration of safety on histologic parameters prior to the initiation of human studies. D-cycloserine (DCS) is a partial NMDA agonist that, along with lurasidone, is being investigated as a treatment for depression.

Background: Peripheral arterial disease (PAD) causes leg muscle damage due to inadequate perfusion and increases cardiovascular events and mortality 2- to 3-fold. It is unclear if PAD is a biomarker for high-risk cardiovascular disease or if skeletal muscle injury harms arterial health. The objective of this work is to test if serum myoglobin levels (myoglobinemia) are a marker of PAD, and if so, whether myoglobin impairs vascular health.

Surface sampling for SARS-CoV-2 RNA detection has shown considerable promise to detect exposure of built environments to infected individuals shedding virus who would not otherwise be detected. Here, we compare two popular sampling media (VTM and SDS) and two popular workflows (Thermo and PerkinElmer) for implementation of a surface sampling program suitable for environmental monitoring in public schools. We find that the SDS/Thermo pipeline shows superior sensitivity and specificity, but that the VTM/PerkinElmer pipeline is still sufficient to support surface surveillance in any indoor setting with stable cohorts of occupants (e.

Background: Over the past 20 years the importance of treatment of people with mental and neurological disorders has greatly increased. Parallel to this development it has become more difficult to attract young physicians to this field. The aim of this study was to examine the development of the number of physicians specialized in the care of patients suffering from neurological, mental and psychosomatic disorders with special consideration of the age structure.

Historically, optimal medical therapy (OMT) has been the primary therapy for acute uncomplicated type B aortic dissection (auTBAD). However, recent data suggest that OMT provides poor long-term results, and aortic remodeling induced by thoracic endovascular aortic repair (TEVAR) may improve survival. This study compares adverse events and survival among auTBAD patients receiving either TEVAR or OMT.

  • Physicians improve their skills quickly during the initial cases of a new procedure, like transcarotid artery revascularization (TCAR), but their improvement plateaus after a while, which highlights the need to identify when proficiency is reached for better patient safety and training.
  • The study focused on analyzing data from over 18,000 TCAR procedures worldwide to find performance measures linked to adverse events, target levels that lower these event rates, and the minimum number of procedures needed to reach these targets.
  • The data collected showed that a diverse group of physicians performed TCAR, with significant proportions of both women and asymptomatic patients participating, emphasizing the need for effective procedural training and performance tracking.
