Article Synopsis
  • Caesarean sections (CS) are the most common major surgery worldwide, but they pose significant risks for maternal mortality, especially in low-resource settings like Malawi, where a study reviewed maternal deaths from 2020 to 2022.
  • The study found that over half of maternal deaths (51.8%) occurred after CS, with women undergoing CS being five times more likely to die compared to those who delivered vaginally, due to causes like postpartum hemorrhage, eclampsia, and infection.
  • Health system issues contributed to these deaths, including delayed treatment and inadequate monitoring, indicating a pressing need to enhance the safety and management of CS in healthcare practices.

Potentially toxic metals and trace elements have been used in Malawi for a long time. However, data on exposure to these elements by susceptible groups like pregnant women and its associations with reproductive health outcomes in Malawi and southern hemisphere is limited. We investigated the concentrations of potentially toxic metals as well as trace elements in pregnant women and assessed the relationship between the levels these elements in maternal blood and sociodemographic factors, dietary habits and birth outcomes.

  • Surgical evacuation of intracerebral hemorrhage (ICH) performed early can lead to better functional outcomes, but doing it too quickly may cause significant risks like postoperative rebleeding.
  • The study developed a 5-point grading scale to assess the severity of intraoperative bleeding during minimally invasive endoscopic techniques for ICH evacuation.
  • Among 142 patients evaluated, results indicated that while early evacuation is associated with increased bleeding scores, it did not lead to worse long-term outcomes or increased postoperative rebleeding.

Aims: Structural heart defects, including congenital ventricular septal defect closure or intracardiac rerouting, frequently require surgical reconstruction using hand-cut patch materials. Digitally modelled patch templates may improve patch fit and reduce outflow tract obstruction, residual defect risk, and conduction system damage. In this study, we benchmarked mixed-reality and a desktop application against a digitalized model of a real implanted patch.

Article Synopsis
  • Endovascular embolization is used as a supportive treatment for meningioma surgery, but evaluating its effectiveness is complicated because MRI scans are done before embolization and after the tumor is removed.
  • Researchers conducted a study using preoperative MRI after embolization to better measure how effective the procedure was, grading the extent of tumor devascularization and analyzing differences based on tumor location and blood supply.
  • The study found that the degree of devascularization correlated well with the volumetric extent of embolization, providing a clearer and more quantifiable way to assess the effectiveness of embolization before surgery.

Climate change poses a significant threat to women's health in sub-Saharan Africa, yet the impact of climate change on maternal health is rarely reported in the region. Using an existing Maternal Surveillance Platform (MATSurvey), we estimated the immediate impact of Cyclone Freddy on maternal health care service indicators in Malawi. We analysed facility-level data for pregnant women up to 42 weeks post-partum using the national MATSurvey database.

Article Synopsis
  • Preoperative embolization is an endovascular procedure that helps in the surgical removal of meningiomas but lacks a standardized system to measure its effectiveness during the procedure.
  • Researchers developed an angiographic grading system to help assess and report the level of tumor devascularization achieved through this process, involving a scale from 0 (no embolization) to 4 (complete embolization).
  • Analysis of data from 80 patients revealed that tumor location and the type of arterial feeders significantly influenced the success of embolization, with certain tumor types showing higher devascularization grades and a low complication rate of 2.5%.*

As a lower middle-income country, Pakistan faces multiple issues that influence the course of healthy ageing. Although there is some understanding of these issues and the objective health outcomes of people in Pakistan, there is less knowledge on the perceptions, experiences, and priorities of the ageing population and their caretakers (hereafter, "stakeholders"). The aim of the study was to identify the needs and priorities of older adults and stakeholders across both urban and rural locations.

Several steps in the abattoir can influence the presence of microbes and associated resistance genes (ARGs) on the animal carcasses used for further meat processing. We investigated how these processes influence the resistome-microbiome of groups of pigs with different on-farm antimicrobial exposure status, from the moment they entered the abattoir until the end of carcass processing. Using a targeted enrichment metagenomic approach, we identified 672 unique ARGs conferring resistance to 43 distinct AMR classes from pooled skin (N = 42) and carcass swabs (N = 63) collected sequentially before, during, and after the slaughter process and food safety interventions.

Article Synopsis
  • The study investigates the potential of e-cigarettes (ECs) to aid pregnant smokers in quitting combustible cigarettes (CCs) and evaluates the characteristics of users who switch to ECs.
  • Approximately 1.5% of pregnant smokers reported using ECs, with around 29.7% of them becoming exclusive EC users; EC initiators tended to have lower socioeconomic status and less access to prenatal care.
  • Findings suggest that while some pregnant smokers who switch to ECs may reduce their CC usage, there is no evidence that this leads to significant health benefits for neonates, indicating that ECs may not be an effective strategy for smoking cessation during pregnancy.
