Article Synopsis
  • CFTR modulator therapies, like elexacaftor, tezacaftor, and ivacaftor, enhance respiratory function and glycaemic control in cystic fibrosis patients, but their effects on pancreatic function in those without diabetes remain uncertain.
  • A case study of an 18-year-old female with cystic fibrosis, who developed hypoglycaemia after starting CFTR therapy, indicates that these treatments may influence glucose regulation even in non-diabetic patients.
  • Managing diet, particularly by avoiding high glycaemic index foods, helped reduce her hypoglycaemic episodes, suggesting CFTR modulator therapies could impact insulin secretion and insulin resistance in cystic fibrosis patients. *

Article Synopsis
  • The study aimed to assess how carbohydrate intake before laparoscopic Roux-en-Y gastric bypass (LRYGB) impacts weight, body composition, and blood sugar levels post-surgery.
  • Patients were divided into two groups based on their carbohydrate intake: moderate (26%-45%) and high (>45%) and evaluated at various times after the surgery.
  • One year later, both groups showed no significant differences in weight or diabetes status, even though the high carbohydrate group consumed fewer calories and carbohydrates after the surgery.

Purpose Of Review: Insulin administration is vitally important to maintain a good glycaemic control in people with type 1 diabetes mellitus (T1DM). The purpose of this review is to give a clinically relevant overview of the newer basal and bolus insulin analogues and to highlight their practicalities of use and advantages in specific categories of patients with T1DM.

Recent Findings: Second-generation rapid-acting insulin analogues (i.

: The aim of insulin replacement in insulin-deficient people (type 1 diabetes, pancreatic causes of diabetes, long-standing type 2 diabetes) is to approximate the physiologic insulin action profile as closely as possible. However, short-acting human insulins start too slow and act too long, causing postprandial hyperglycemia and delayed hypoglycemia, while the insulin action profile of long-acting human insulins is too variable in duration and strength of action, leading to insufficient basal insulin covering and peak insulin levels after injection causing early nocturnal hypoglycemia. Insulin analogues were designed to overcome these shortcomings.

Background: Radiofrequency ablation (RFA), combined with endoscopic resection, can be used as a primary treatment for low grade dysplasia, high grade dysplasia, and early esophageal adenocarcinoma (EAC) in Barrett's esophagus (BE). The aim of the Belgian RFA registry is to capture the real-life outcome of endoscopic therapy for BE with RFA and to assess efficacy and safety outside study protocols, in the absence of reimbursement.

Patients And Methods: Between February 2008 and January 2017, data from 7 different expert centers were prospectively collected in the registry.
