Background And Objectives: Inability to obtain timely medications is a patient safety concern that can lead to delayed or incomplete treatment of illness. While there are many patient and system factors contributing to postdischarge medication nonadherence, availability and insurance-related barriers are preventable.

Purpose: To implement a systematic process ensuring review of discharge prescriptions to ensure availability and resolve insurance barriers before patient discharge.
