Therapeutic augmentation of ketogenic diet with a sodium-glucose cotransporter 2 inhibitor in a super-refractory status epilepticus patient.

Background: A ketogenic diet (KD) may have a role in treating patients in super-refractory status epilepticus (SRSE). Sodium-glucose cotransporter 2 (SGLT2) inhibitors have a risk of ketoacidosis that could facilitate induction of KD.

Case Summary: A 42-year-old with a history of drug resistant epilepsy developed SRSE requiring several pharmacological interventions during her hospital course including the initiation of KD that failed.

Fosphenytoin-induced purple glove syndrome: A case report.

Background: Purple glove syndrome (PGS) is a poorly understood severe adverse drug reaction that is typically associated with intravenous phenytoin administration. Although fosphenytoin is thought to circumvent this risk of PGS, we reveal a rare case of PGS in a patient treated with fosphenytoin therapy.

Case Summary: A 71-year-old male with history of epilepsy was admitted for seizures and traumatic brain injury and intravenous fosphenytoin and levetiracetam were initiated.

A quantitative EEG method for detecting post clamp changes during carotid endarterectomy.

Objective: To apply univariate and multivariate analyses of EEG spectral features in order to detect subtle post-clamp changes during carotid endarterectomy, and to devise a data-driven, multivariate classification method for rapid, real-time detection of small post-clamp EEG changes.

Methods: We used intraoperative EEG records of 27 patients who underwent carotid endarterectomy (CEA) and did not undergo a shunt per surgeon's and technologist's visual inspection of the electro-encephalogram (EEG). Furthermore, these records could not be flagged with r-sBSI (revised spatial brain symmetry index) or r-tBSI (revised temporal brain symmetry index) qEEG (quantitative EEG) metrics.
