Article Synopsis
  • A standardized medication plan (MPlan) is essential for improving medication safety and adherence, especially during transitions in patient care, but Austria currently lacks such standardization.
  • In a study involving 512 patients undergoing elective coronary angiography, only 55.1% had a medication document (MDoc) at hospital admission, with many missing crucial information and a significant portion lacking MDocs altogether.
  • A high discrepancy rate of 64.4% was found between the MPlans and patients' actual medications, indicating potential risks to the effectiveness and safety of their treatment regimens.
