Objective: Ilizarov technology is highly effective in addressing complex orthopedic challenges. This study aims to describe our experience with distraction osteogenesis in managing bone tumors in the lower extremity, focusing on composite bone defects and associated complications.
Methods: A retrospective clinical study was conducted to analyze patients with primary bone tumors who underwent distraction osteogenesis using the Ilizarov method from 2010 to 2020.

An 8-year-old boy was shot in his right foot and ankle, which resulted in soft tissue lesions of the medial malleolus and lateral calcaneus, fracture of the medial malleolus, and partial loss of the calcaneus. We designed a distally based sural flap and a posterior tibial perforator flap for reconstruction of soft tissue defects at the lateral aspect of the calcaneus and the medial malleolus, respectively. Both flaps survived successfully, and the boy had a normal gait during follow-up.

Results showed that Terzis grooming test scores were significantly increased at 6 months after treatment, the latency of C5-6 motor evoked potential was gradually shortened, and the amplitude was gradually increased. The rate of C3 instead of C5 and the C4 + phrenic nerve instead of C6 myelinated nerve fibers crossing through the anastomotic stoma was approximately 80%.
