Article Synopsis
  • The study aimed to compare outcomes of laparoscopic primary repair (LPR) versus LPR combined with highly selective vagotomy (LPR-HSV) in patients with perforated duodenal ulcers.
  • A total of 184 patients were analyzed, revealing that LPR-HSV had a longer operation time but significantly shorter hospital stays and quicker recovery in starting soft diets compared to LPR.
  • The findings suggest that LPR-HSV could be a safe option for patients at high risk of ulcer recurrence, without significant differences in complication rates.

Anastomotic leakage is one of the common causes of serious morbidity and death after gastrectomy. The use of surgical treatment for leakage decreased due to the development of nonsurgical management. However, if nonsurgical management fails to control the spread of intra-abdominal infection, emergency surgical treatment is required.

Article Synopsis
  • - A 65-year-old woman developed skin changes, including hyperpigmentation and thickening, in her breast 10 years after having breast-conserving surgery and radiotherapy for cancer, raising concerns about several potential conditions including post-irradiation morphea and recurrent cancer.
  • - Despite magnetic resonance imaging (MRI) showing no abnormalities, the patient experienced pain and worsening skin symptoms, leading to a simple mastectomy, which revealed oleoma and post-radiation fibrosis.
  • - The case emphasizes the challenges in differentiating between post-irradiation morphea and oleoma due to the patient's complex medical history, highlighting the need for a multidisciplinary approach to ensure accurate diagnosis.

Small intestinal malignant tumor accounts for about 3% of all malignant tumors in the gastrointestinal tract, among which 13% are leiomyosarcoma (LMS). In addition, epithelioid LMS is of very rare occurrence. As small intestinal malignant tumors are initially asymptomatic and nonspecific, diagnosis is often delayed, and this can lead to large tumor at the time of detection and lead to intussusception.

Background: The aim of this study was to compare the 1 year incidence of Petersen's hernia between individuals who were treated with the jejunal mesentery fixing (Mefix) method and those with the closure of Petersen's space method.

Material And Methods: We retrospectively collected clinical data of patients who underwent gastrectomy for gastric cancers with the closure of Petersen's space defect ( = 49) and Mefix ( = 26). The Mefix method was performed by fixing the jejunal mesentery (jejunojejunostomy below 30 cm) to the transverse mesocolon using nonabsorbable barbed sutures.

Toxocara canis is an important roundworm of canids and a fearsome animal parasite of humans. Human infections can lead to syndromes called visceral larva migrans (VLM), ocular larva migrans, neurotoxocariasis, and covert toxocariasis. VLM is most commonly diagnosed in children younger than 8 years of age, but adult cases are relatively frequent among those infected by ingesting the raw tissue of paratenic hosts in East Asia.

Ultrasmallsuperparamagnetic iron oxide (USPIO) has been suggested to be a negative MR contrast agent to detect metastatic lymph nodes. Previously reported studies have evaluated the diagnostic performance of USPIO-enhanced MR lymph node imaging based on signal intensity. In this study, we investigate the specific performance of three different parametric approaches (normalized signal intensity, R * and susceptibility) using 3D multi-echo gradient echo to quantify the USPIO particles in lymph nodes.

Extranigral non-motor signs precede the first motor manifestations of Parkinson's disease by many years in some patients. The presence of α-synuclein deposition within colon tissues in patients with Parkinson's disease can aid in identifying early neuropathological changes prior to disease onset. In the present study, we evaluated the roles of non-motor symptoms and signs and imaging biomarkers of nigral neuronal changes and α-synuclein accumulation in the colon.

We have attempted to reduce blood use during the perioperative period to avoid complications associated with blood transfusions in pediatric patients undergoing open-heart surgery. We retrospectively reviewed clinical data of patients who underwent open-heart surgery (age < 15 years, body weight ≤ 30 kg) from January 2012 to October 2013. Our strategy to reduce transfusion volume included: (1) shortening the length of cardiopulmonary bypass (CPB) circuit, and adding red blood cells (RBC) to CPB circuit priming solution when preoperative hematocrit was ≤30%; (2) routine modified ultrafiltration in all patients; and (3) restricting RBC transfusions during postoperative period, given when hematocrit was ≤25%.
