Background & objectives Surgical site infections (SSIs) are among the most prevalent healthcare-associated infections (HCAIs). They cause significant morbidity, leading to excess health expenditures and increased length of hospital stay. Despite a high population burden, data on post-discharge SSIs is lacking from low-and middle-income countries (LMICs).

Background: During the COVID-19 pandemic, there were reports of a shortage of ventilators and oxygen supply, particularly in resource-limited settings. We report the preliminary evaluation of a non-invasive positive end-expiratory pressure (PEEP) mask in hospitalized non-critically ill patients with COVID-19.

Methods: We randomly assigned hospitalized adult patients with confirmed COVID-19 infection and requiring greater than 40% supplemental oxygen to either standard care oxygen delivery (control) or via Materialise passive non-invasive PEEP device mask (intervention; Belgium).

Unlabelled: Gender disparity in Critical Care Medicine (CCM) persists globally, with women being underrepresented. Female Intensivists remain a minority, facing challenges in academic and leadership positions at the workplace and within academic societies. The Indian Society of Critical Care Medicine (ISCCM) recognized the need for addressing issues related to gender parity and constituted its first Diversity Equity and Inclusion (DEI) Committee in 2023.

Article Synopsis
  • * The study focused on creating a set of expert guidelines for managing difficult airways in critically ill adults, specifically those with physiologically challenging conditions like obesity and pregnancy.
  • * An international group of airway management specialists used the Delphi method, which involved multiple rounds of surveys, to achieve consensus on 53 out of 61 proposed statements regarding best practices.
  • * Key recommendations included forming a robust intubation team, using videolaryngoscopy, optimizing patient conditions before intubation, and carefully monitoring the patient's status post-intubation to improve overall outcomes.

  • Central line-associated bloodstream infection (CLABSI) rates in Latin American ICUs are significantly higher than in high-income countries, prompting a need for intervention.
  • The INICC multidimensional approach, which includes an 11-component bundle, was implemented across 122 ICUs in nine Asian countries, resulting in a substantial decrease in CLABSI rates from 16.64 to 2.18 over 29 months.
  • The intervention not only reduced CLABSI rates by 87% but also significantly lowered the all-cause in-ICU mortality rate from 13.23% to 10.96%.

Unlabelled: Skin mottling has been found to be useful as a marker of peripheral hypoperfusion in shock in studies performed on fair-skinned patients. Whether skin mottling may be less apparent in dark-skinned patients, thus limiting its value in this patient population has not been studied. Jog et al.

Unlabelled: End-of-life care (EOLC) exemplifies the joint mission of intensive and palliative care (PC) in their human-centeredness. The explosion of technological advances in medicine must be balanced with the culture of holistic care. Inevitably, it brings together the science and the art of medicine in their full expression.

Purpose: To share our clinical experience with the diagnosis and management of children with hematolymphoid malignancies presenting with epilepsia partialis continua (EPC) as a sequelae of measles infection.

Materials And Methods: In December 2022, a series of children in our hemato-oncology unit presented with focal status epilepticus with no conclusive evidence pointing toward any underlying etiology. One such child had a typical measles rash a few weeks before the onset of this focal status epilepticus.

This narrative review explores the evolving role of artificial intelligence (AI) in haemodynamic monitoring, emphasising its potential to revolutionise patient care. The historical reliance on invasive procedures for haemodynamic assessments is contrasted with the emerging non-invasive AI-driven approaches that address limitations and risks associated with traditional methods. Developing the hypotension prediction index and introducing CircEWS and CircEWS-lite showcase AI's effectiveness in predicting and managing circulatory failure.

Background And Aims: The International Normalised Ratio (INR), which assesses the loss of procoagulant factors in the extrinsic pathway, fails to evaluate the coagulation abnormalities comprehensively after a major liver resection, which often leads to reduced synthesis of procoagulant and anticoagulant-factors. This study was conducted with an aim to study the trend and compare the results of routine coagulation tests and thromboelastography (TEG) during the perioperative period in patients undergoing major liver resections (≥3 segments).

Methods: Twenty-five patients who underwent a major liver resection were enrolled.

Background: There is conflicting evidence on association between quick sequential organ failure assessment (qSOFA) and sepsis mortality in ICU patients. The primary aim of this study was to determine the association between qSOFA and 28-day mortality in ICU patients admitted for sepsis. Association of qSOFA with early (3-day), medium (28-day), late (90-day) mortality was assessed in low and lower middle income (LLMIC), upper middle income (UMIC) and high income (HIC) countries/regions.

Puneet Kaur Somal  Classification of breast cancer into different molecular subtypes has important prognostic and therapeutic implications. The immunohistochemistry surrogate classification has been advocated for this purpose. The primary objective of the present study was to assess the prevalence of the different molecular subtypes of invasive breast carcinoma and study the clinicopathological parameters in a tertiary care cancer center in rural North India.

Background: Identify urinary catheter (UC)-associated urinary tract infections (CAUTI) incidence and risk factors (RF) in 235 ICUs in 8 Asian countries: India, Malaysia, Mongolia, Nepal, Pakistan, the Philippines, Thailand, and Vietnam.

Methods: From January 1, 2014, to February 12, 2022, we conducted a prospective cohort study. To estimate CAUTI incidence, the number of UC days was the denominator, and CAUTI was the numerator.

Article Synopsis
  • * It focuses on adult patients with upper aerodigestive tract squamous cell carcinoma who had surgery, analyzing data from 245 patients between July and December 2019.
  • * Findings indicate that surgical site infections (SSI) significantly increase the risk of UHR and delay in adjuvant treatment, also correlating with poorer disease-free survival rates among affected patients.

  • Out of 150,142 patients, a total of 1514 CLABSIs were recorded, with an overall infection rate of 5.08 per 1000 central line days, highest in femoral and temporary hemodialysis catheters.
  • Key risk factors for CLABSI included longer hospital stays before infection, tracheostomy use, hospitalization type, and facility ownership, particularly in publicly-owned and lower-middle-income country facilities.

Background And Aims: The analgesic role of gabapentinoids following thoracic surgeries is not clear. In this study, we evaluated the benefits of gabapentinoids for pain management in patients undergoing thoracic onco-surgery in terms of opioids and non-steroidal anti-inflammatory drugs (NSAIDs) sparing effect. We also compared pain scores (PSs), number of days of active surveillance by the acute pain service team, and side effects associated with gabapentinoids.

Unlabelled: Hemodynamic assessment along with continuous monitoring and appropriate therapy forms an integral part of management of critically ill patients with acute circulatory failure. In India, the infrastructure in ICUs varies from very basic facilities in smaller towns and semi-urban areas, to world-class, cutting-edge technology in corporate hospitals, in metropolitan cities. Surveys and studies from India suggest a wide variation in clinical practices due to possible lack of awareness, expertise, high costs, and lack of availability of advanced hemodynamic monitoring devices.
