It remains unclear why unilateral proximal carotid artery occlusion (UCAO) causes benign oligemia in mice, yet leads to various outcomes (asymptomatic-to-death) in humans. We hypothesized that inhibition of nitric oxide synthase (NOS) both transforms UCAO-mediated oligemia into full infarction and expands pre-existing infarction. Using 900 mice, we i) investigated stroke-related effects of UCAO with/without intraperitoneal administration of the NOS inhibitor (NOSi) N-nitro-L-arginine methyl ester (L-NAME, 400 mg/kg); ii) examined the rescue effect of the NO-donor, molsidomine (200 mg/kg at 30 minutes); and iii) tested the impact of antiplatelet medications.

Based on the hypothesis that hyperuricemia is a modifiable risk factor for chronic kidney disease (CKD) progression, there is an expectation that urate-lowering therapy (ULT) could delay the progression of CKD. Here, we investigated changes in kidney function and the association of the serum uric acid (sUA) level and kidney function during ULT in patients with gout. To do this we conducted post-hoc analysis on patients who received ULT with either febuxostat or allopurinol for more than six months in the CARES trial.

Article Synopsis
  • Advanced biliary tract cancer (BTC) patients may benefit from fluoropyrimidine-based chemotherapy, but no direct comparisons between different regimens had been previously conducted.
  • An individual patient-level meta-analysis was performed using data from two clinical trials to evaluate overall survival (OS) across different chemotherapy regimens, including nal-IRI plus 5-FU/LV, mFOLFOX, and mFOLFIRI.
  • Results showed that nal-IRI plus 5-FU/LV significantly improved OS compared to both mFOLFOX and mFOLFIRI, with notable side effects differing among regimens; thus, considering both effectiveness and safety is essential in treatment decisions.

Article Synopsis
  • This study examined how non-traditional lipid profiles, specifically the LDL/HDL ratio, affect the risk of vascular events within a year in stroke patients already on statins and with low LDL-C levels.
  • The analysis included 7028 patients with acute ischemic strokes and found a significant association between higher LDL/HDL ratios and increased risk of recurrent stroke, heart attack, or death after adjusting for other variables.
  • The results suggest that even with low LDL-C levels due to statin use, monitoring the LDL/HDL ratio is important to assess residual risk after a stroke.

We investigated the clinical and radiologic predictors of unruptured symptomatic intracranial artery dissection (IAD) outcomes. Unruptured symptomatic IAD patients who underwent vessel wall magnetic resonance imaging (VW-MRI) and time-of-flight magnetic resonance angiography (TOF-MRA) within 1 month after symptom onset, followed for over 12 months were included. Baseline features predicting the clinical outcome of recurrent symptoms and radiologic outcomes of aneurysmal dilatation and occlusion were analyzed using logistic regression analysis.

Article Synopsis
  • A multi-center MRI study investigated how white matter hyperintensity (WMH) affects recovery after stroke, focusing on different severity levels of ischemic stroke.
  • Higher WMH levels were linked to worse outcomes three months later, but the impact varied based on initial stroke severity: mild strokes showed a dose-dependent effect while moderate-to-severe strokes had a threshold effect.
  • The study found that WMH impacted 3-month recovery more significantly in those with mild strokes, suggesting that increased WMH burden worsens recovery, but its effect is less pronounced in more severe strokes.

Article Synopsis
  • The study explored whether evogliptin, a dipeptidyl peptidase-4 inhibitor, could slow the progression of aortic stenosis (AS) in 228 patients over 96 weeks, measuring changes in aortic valve calcium volume (AVCV).
  • Results showed no significant reduction in AVCV for either dosage of evogliptin compared to placebo; however, evogliptin groups had significantly lower active calcification volume at 48 weeks using F-sodium fluoride PET.
  • Although evogliptin did not appear to protect against AV calcification, the study suggests the potential for longer-term benefits, indicating a need for larger trials to confirm these findings.

Background: Older adults are at a higher risk of severe adverse drug events (ADEs) because of multimorbidity, polypharmacy, and lower physiological function. This study aimed to determine whether polypharmacy, defined as the use of ≥ 5 active drug ingredients, was associated with severe ADEs in this population.

Methods: We used ADE reports from the Korea Institute of Drug Safety and Risk Management-Korea Adverse Event Reporting System Database, a national spontaneous ADE report system, from 2012 to 2021 to examine and compare the strength of association between polypharmacy and severe ADEs in older adults (≥ 65 years) and younger adults (20-64 years) using disproportionality analysis.

Article Synopsis
  • In a study of patients aged 60 and older with patent foramen ovale (PFO) and cryptogenic stroke from South Korea, researchers aimed to determine if PFO closure helps reduce the risk of recurrent strokes compared to medical therapy alone.
  • The study included 437 patients, with 37% undergoing PFO closure; results showed that PFO closure significantly lowered the risk of subsequent ischemic strokes or TIAs over a follow-up period of about 3.9 years.
  • The findings suggest that elderly patients with high-risk PFO have a substantial recurrence rate of strokes, which PFO closure can effectively reduce.

We selectively improved the viewing angle characteristics and light extraction efficiency of blue thermally activated delayed fluorescence (TADF) organic light-emitting diodes (OLEDs) by tailoring a nanofiber-shaped SiN layer, which was used as an internal scattering layer. The diameter of the polymer nanofibers changed according to the mass ratio of polyacrylonitrile (PAN) and poly(methyl methacrylate) (PMMA) in the polymer solution for electrospinning. The SiN nanofiber (SNF) structure was fabricated by etching an SiN film using the PAN/PMMA nanofiber as a mask, making it easier to adjust parameters, such as the diameter, open ratio, and height, even though the SNF structure was randomly shaped.
