Pediatric-specific safety data are required during development of pharmaceutical agents. Retrospective studies can leverage real-world data to assess safety and effectiveness in children where prospective, controlled studies are not feasible. A retrospective cohort study combined data from Pediatric Health Information Systems (PHIS) and medical records to evaluate the safety and effectiveness of piperacillin/tazobactam (P/T) in pediatric patients with hospital-acquired pneumonia (HAP).

Article Synopsis
  • * Gabapentin was given to 22.9% of infants studied, showing a trend of increased use over time, and those who received it tended to have longer hospital stays and chest tube durations.
  • * Results indicated a slight decrease in benzodiazepine use for infants taking gabapentin, but no significant change in opioid use, suggesting that further research is needed to fully understand gabapentin's role in postoperative care.

Background: Deimplementing overused health interventions is essential to maximizing quality and value while minimizing harm, waste, and inefficiencies. Three national guidelines discourage continuous pulse oximetry (SpO) monitoring in children who are not receiving supplemental oxygen, but the guideline-discordant practice remains prevalent, making it a prime target for deimplementation. This paper details the statistical analysis plan for the Eliminating Monitor Overuse (EMO) SpO trial, which compares the effect of two competing deimplementation strategies (unlearning only vs.

Diastolic dysfunction after repair for Tetralogy of Fallot (TOF) is associated with adverse long-term outcomes. Right atrial (RA) mechanics as a proxy of right ventricular (RV) diastolic function in the early post-operative period after surgical repair for TOF has not been reported. We sought to evaluate RA and RV strain prior to hospital discharge after TOF repair and to identify important patient factors associated with strain using a machine learning method.

Using continuous pulse oximetry (cSpO ) to monitor children with bronchiolitis who are not receiving supplemental oxygen is a form of medical overuse. In this longitudinal analysis from the Eliminating Monitor Overuse (EMO) study, we aimed to assess changes in cSpO overuse before, during, and after intensive cSpO -deimplementation efforts in six hospitals. Monitoring data were collected during three phases: "P1" baseline, "P2" active deimplementation (all sites engaged in education and audit and feedback strategies), and "P3" sustainment (a new baseline measured after strategies were withdrawn).

Background The impact of home monitoring on unanticipated interstage readmissions in infants with hypoplastic left heart syndrome has not been previously studied. We sought to examine the association of our institution's Infant Single Ventricle Management and Monitoring Program (ISVMP) with readmission frequency, cumulative readmission days, and readmission illness severity and to identify patient-level risk factors for readmission. Methods and Results We performed a retrospective single-center cohort study comparing infants with hypoplastic left heart syndrome enrolled in ISVMP (December 2010-December 2019) to historical controls (January 2007-November 2010).

Article Synopsis
  • Obstructed total anomalous pulmonary venous connection (TAPVC) is a critical heart defect that requires urgent care after birth, and understanding which patients have severe obstruction can help in planning deliveries.
  • The study analyzed 29 neonates diagnosed prenatally with TAPVC, focusing on a new measurement called the pulmonary venous variability index (PVVI) and Doppler velocities of a vertical vein to predict postnatal obstruction severity.
  • Results showed that lower PVVI values and specific vertical vein velocities were linked to a higher risk of severe pulmonary vein obstruction after birth, indicating these metrics could be valuable for managing affected infants.
