Article Synopsis
  • - This study investigates how pre-existing renal impairment affects outcomes for patients with upper tract urothelial carcinoma (UTUC) who underwent radical nephroureterectomy (RNU), utilizing data from an international registry.
  • - Out of 1393 patients analyzed, those with chronic kidney disease (CKD) had worse disease-free survival (DFS) compared to patients with normal renal function; CKD was linked to a higher likelihood of poor outcomes.
  • - The study concludes that while pre-existing kidney issues don't impact overall survival (OS), they do correlate with worse DFS, highlighting the importance of renal health in treatment planning for UTUC patients.

Article Synopsis
  • - Over the last 30 years, miniaturized percutaneous nephrolithotomy (mPCNL) has gained popularity due to its potential to reduce complications compared to standard procedures, but there's still debate over best practices and management strategies that hinders wider use.
  • - An international panel of urology experts developed a consensus document on mPCNL to provide a comprehensive framework for practice, which included reviewing literature, identifying research gaps, and conducting surveys to gather expert opinions.
  • - The study revealed 58 key questions on mPCNL practices, and consensus was achieved on 30 questions, affirming mPCNL as an effective technique for treating upper urinary tract stones in both adults and children.

Background: The study assesses the decisional regret following Shared Decision-making (SDM) in patients selecting either early ureteroscopic lithotripsy (URSL) or medical expulsive therapy (MET) for ureteric stones ≤ 1 cm, with the aim to evaluate their decisional Conflict, satisfaction, and regret regarding their opted treatment choices.

Methods: Adults aged more than 18 years with one stone up to 1 cm in either ureter were included. After SDM, the patients were allocated into their opted group viz.

Background: Since its initial description the prostate biopsy technique for detection of prostate cancer (PCA) has constantly evolved. Multiparametric magnetic resonance imaging (mpMRI) has been proven to have a sensitivity exceeding 90% to detect the index lesion. This narrative review discusses the evidence around several biopsy strategies, especially in the context of patients that might be eligible for focal therapy.

(1) Introduction: Diagnostic ureteroscopy (URS) is an important component in the workup of upper tract urothelial carcinoma (UTUC). Whether URS was associated with increased recurrence in the bladder was not fully concluded. The current study aimed to evaluate the implication of URS on the incidences of intravesical recurrence following radical nephroureterectomy (RNU) in non-metastatic UTUC patients without prior history of bladder cancer via multi-institutional data.

Background: The evidence regarding perioperative adjuvant chemotherapy and personalized surveillance strategies for upper tract urothelial carcinoma is limited.

Objective: To evaluate whether adjuvant gemcitabine containing chemotherapy affects the oncological outcomes of advanced upper tract urothelial carcinoma (UTUC).

Methods: The CROES-UTUC registry is an observational, international, multi-center study on patients diagnosed with UTUC.

LUTS and voiding dysfunctions are prevalent in urology clinics, with uroflowmetry and IPSS as the prevailing diagnostic methods. Nevertheless, objective assessment can be constrained by age, gender, and variability in the test conditions. Portable (home) uroflowmetry addresses these limitations, allowing for more natural urinary flow recordings beyond clinic confines.

Kidney-sparing surgery (KSS) for upper tract urothelial cancer (UTUC) has gained increasing interest recently. However, there is limited contemporary data regarding the role of KSS in ureteral urothelial carcinoma. Therefore, we investigated the survival outcomes of ureteral urothelial carcinoma after KSS from a large, prospective international UTUC registry.

: Urinary incontinence diminishes quality of life, and its severity can be worsened by mobility impairments. This study explored the link between urinary incontinence, osteoarthritis, and back musculoskeletal system disorders, considering pain, mobility issues, and daily activity difficulties. : This cross-sectional study included respondents aged ≥ 15 years from the 2008 Turkish Health Studies Survey (n = 13,976).

Purpose: To assess the safety and efficacy of super-mini PCNL (SMP, 14 Fr) when compared to standard PCNL (sPCNL, 24-30 Fr) in the management of renal calculi of size ranging from 1.5 to 3 cm.

Methods: From February 2021 to January 2022, a total of 100 patients were randomized to either SMP group or sPCNL group in a 1:1 ratio (50 in each group) using computer-generated simple randomization.

Article Synopsis
  • Non-obstructive azoospermia (NOA) is a prevalent and complex issue with varied treatment options and no definitive guidelines, leading to differing management practices internationally.
  • A comprehensive survey with 336 responses from specialists in 49 countries explored current medical and surgical strategies for NOA, analyzing results against existing guidelines and offering expert recommendations.
  • Key findings included diverse approaches to hormonal therapy, significant variation in sperm retrieval success rates, and differing protocols around varicocele repair and follicle-stimulating hormone cutoff levels for sperm retrieval.

Article Synopsis
  • Diagnostic ureteroscopy (dURS) is a valuable optional procedure for assessing patients with upper tract urothelial carcinoma (UTUC), particularly for obtaining tissue samples for histology.
  • Data from the CROES-UTUC registry, analyzing 2380 patients, showed that dURS with biopsy was performed in 31.6% of cases, yielding adequate samples for diagnosis in 83.5% of situations, although about one-third of patients were under-staged.
  • High-grade biopsies effectively predicted high-grade and high-risk diseases with high accuracy, but low-grade tumors showed a significant number of patients with undetected high-risk disease, indicating a need for better diagnostic methods.

Article Synopsis
  • This systematic review and meta-analysis investigates whether varicocele repair (VR) can lower sperm DNA fragmentation (SDF) and seminal oxidative stress (measured by malondialdehyde or MDA) in infertile men.
  • An analysis of 29 studies involving 1,491 men shows a significant reduction in SDF post-VR, regardless of the surgical technique used and the method of SDF testing.
  • The findings also indicate a notable decrease in seminal MDA levels following VR, supporting the effectiveness of varicocele repair in improving reproductive parameters in affected men.

Introduction: The drug classes of α-adrenoceptor antagonists, 5α-reductase inhibitors, and phosphodiesterase type 5 inhibitors are guideline-recommended treatments of lower urinary tract symptoms suggestive of benign prostatic hyperplasia; muscarinic receptor antagonists and β-adrenoceptor agonists are also recommended if storage symptoms are insufficiently addressed with one of the other three drug classes.

Areas Covered: We provide a narrative review (no formalized literature searches performed) of the tolerability of these drug classes with emphasis on the more recently introduced medications, on combination treatment, and on more lately emerging risks.

Expert Opinion/commentary: The tolerability profiles are distinct between drug classes but, with few exceptions, similar within a drug class.

Article Synopsis
  • The study investigates global practices and challenges in using sperm DNA fragmentation (SDF) assays, which can affect male reproductive potential, as outlined in the latest WHO manual.
  • A survey of 436 reproductive clinicians revealed that the most popular SDF assay is TUNEL, with a significant influence from availability on their choices.
