Article Synopsis
  • The eTICI score, previously validated for large proximal artery occlusions (pLAOs), has not been fully tested for distal medium vessel occlusions (DMVOs), prompting this study to assess its reliability in DMVO cases.
  • Researchers conducted a retrospective analysis of data from a stroke center, where two specialists independently evaluated angiograms, comparing their eTICI score assessments.
  • Results showed that eTICI scores for DMVO demonstrated almost-perfect agreement among raters, comparable to pLAO cases, indicating that eTICI can be reliably used for DMVOs, but more studies are needed to explore its connection to clinical outcomes.

Article Synopsis
  • This study aimed to compare the effectiveness of rescue therapy (RT) versus medical management (MM) in improving functional outcomes for patients who experienced a failed mechanical thrombectomy (MT).
  • Analyzing data from 2011 to 2021, the research focused on patients with large vessel occlusions (LVOs) and assessed disability at 90 days using the modified Rankin Scale (mRS).
  • Results showed that RT led to better functional independence, lower rates of symptomatic intracranial hemorrhage, and decreased 90-day mortality compared to MM, suggesting that RT might be a more effective treatment strategy for these patients.

Background: Carotid free-floating thrombus (CFT) is a rare cause of stroke describing an intraluminal thrombus that is loosely associated with the arterial wall and manifesting as a filling defect fully surrounded by flow on vascular imaging. Unfortunately, there is no clear consensus among experts on the ideal treatment for this pathology.

Methods: Retrospective analysis of acute ischemic stroke (AIS) and transient ischemic attack (TIA) patients diagnosed with CFT on computed tomography angiogram (CTA) between January 2015-March 2023.

Article Synopsis
  • Early changes in endovascular therapy (EVT) techniques for ischemic stroke can impact reperfusion outcomes, particularly after initial failed attempts.
  • Switching from stent retriever (SR) to combined technique (CT) after two unsuccessful passes significantly improves chances of achieving complete/near-complete reperfusion (eTICI 2c-3).
  • Adjusting strategy from contact aspiration (CA) to CT also shows improved outcomes, while changing from CA to SR had no significant benefits, highlighting the importance of flexible approaches in EVT.

Background And Importance: Neurointervention is a very competitive specialty in the United States due to the limited number of training spots and the larger pool of applicants. The training standards are continuously updated to ensure solid training experiences. Factors affecting candidate(s) selection have not been fully established yet.

Diffuse leptomeningeal glioneuronal tumor is unique for communicating hydrocephalus, diffuse leptomeningeal enhancement, cystic changes, absence of tumor cells in cerebral spinal fluid, and a cell population of both glial and neuronal copositivity. It has likely been misdiagnosed as mixed glioneuronal tumors, oligodendrogliomas, and neuroepithelial tumors. Children with signs of this tumor are often worked up for infection, rheumatologic disease, or disseminated primary malignancy, resulting in unnecessary testing and treatment.
