Ramadan intermittent fasting can pose challenges and risks for some groups of patients. Based on a narrative literature review and our clinical expertise, we provide practical guidance for clinicians managing patients with gastrointestinal and hepatobiliary conditions who wish to fast during Ramadan. Following the established International Diabetes Federation and Diabetes and Ramadan International Alliance risk stratification framework, we categorised patients' risk as low or moderate, high, or very high.

This case report discusses the diagnosis and management of a 25-year-old pregnant patient presenting with persistent tachycardia. The patient, with a past medical history of thyroiditis, polycystic ovarian syndrome, and obesity, was admitted due to palpitations and was diagnosed with atrial tachycardia. Despite medical management with metoprolol, adenosine, digoxin, and flecainide, the tachycardia persisted, necessitating discussion about cardiac ablation.

Accurate counting of mosquito larval populations is essential for maintaining optimal conditions and population control within rearing facilities, assessing disease transmission risks, and implementing effective vector control measures. While existing methods for counting mosquito larvae have faced challenges such as the impact on larval mortality rate, multiple parameters adjustment and limitations in availability and affordability, recent advancements in artificial intelligence, particularly in AI-driven visual analysis, hold promise for addressing these issues. Here, we introduce LarvaeCountAI, an open-source convolutional neural network (CNN)-based tool designed to automatically count Culex annulirostris mosquito larvae from videos captured in laboratory environments.

This article introduces what we believe to be a novel receiver design featuring a compact, wide-field-of-view receiver and bidirectional gain medium that enables multiple receivers to charge simultaneously. The receiver was tested with a transmitter comprising a bidirectional broadband semiconductor optical amplifier and two wavelength division multiplexing filters. The receiver incorporates a ball-lens retroreflector equipped with anti-reflection and highly reflective coatings on its front and rear surfaces, respectively.

Monitoring the flight behaviour of mosquitoes is crucial for assessing their fitness levels and understanding their potential role in disease transmission. Existing methods for tracking mosquito flight behaviour are challenging to implement in laboratory environments, and they also struggle with identity tracking, particularly during occlusions. Here, we introduce FlightTrackAI, a robust convolutional neural network (CNN)-based tool for automatic mosquito flight tracking.

Article Synopsis
  • - This study investigates differences in clinical features and outcomes of non-acute myocardial infarction-associated cardiogenic shock (nAMI-CS) between Hispanic and non-Hispanic patients in the U.S. using data from hospitalizations between 2018 and 2020.
  • - Out of 8607 hospitalizations, 832 (9.6%) were Hispanic, who presented as younger and with more co-morbidities like diabetes and heart failure, while having a slightly higher in-hospital mortality rate (18.6% vs. 17%).
  • - Hispanic patients experienced longer hospital stays and higher treatment costs compared to non-Hispanic patients, indicating the need for further research to explore the reasons behind these health disparities.

The symptoms of COVID-19 included fever with or without respiratory syndrome, but patients subsequently developed pulmonary abnormalities. Exposure to air pollution, meanwhile, is associated with complications such as acute respiratory inflammations, asthma attack, and deaths from cardiorespiratory disease. To analyze the association of the air quality index (AQI), ambient air pollutants (PM, SO and O) and meteorological parameters (temperature and relative humidity [RH]) with COVID-19 incidence and mortality, a retrospective study was conducted to examine COVID-19 infection, meteorological parameters, ambient air quality and ambient air pollutants in Dammam from 1 January to 30 April 2021.

Leptospirosis is a global health concern, particularly in tropical regions, with clinical symptoms varying from mild fever to severe organ dysfunction. We present a case of a 57-year-old male with septic shock and acute kidney injury due to acute leptospirosis. The patient's rapid progression to shock within a day of generalized symptoms was unusual.

Mosquito-borne diseases such as malaria, dengue, Zika, and chikungunya cause significant morbidity and mortality globally, resulting in over 600,000 deaths from malaria and around 36,000 deaths from dengue each year, with millions of people infected annually, leading to substantial economic losses. The existing mosquito control measures, such as long-lasting insecticidal nets (LLINs) and indoor residual spraying (IRS), helped to reduce the infections. However, mosquito-borne diseases are still among the deadliest diseases, forcing us to improve the existing control methods and look for alternative methods simultaneously.

Article Synopsis
  • GLP-1 receptor agonists are commonly used in diabetes and obesity treatment, but their use may increase the risk of gastroparesis and aspiration during surgical or endoscopic procedures, leading to recent guidelines recommending their discontinuation before surgery.
  • A study at BronxCare Health System analyzed data from 306 patients, comparing those on clear liquid diets versus regular diets before endoscopic procedures, focusing on gastric residue and aspiration risks.
  • Results indicated that patients on clear liquid diets had significantly less gastric residue compared to those on regular diets, suggesting that a 24-hour liquid diet is safe before such procedures, with no reported complications.

Gastrointestinal diseases are increasing in global prevalence. As a result, the contribution to both mortality and healthcare costs is increasing. While interventions utilizing scoping techniques or ultrasound are crucial to both the timely diagnosis and management of illness, a few limitations are associated with these techniques.

Objectives: To evaluate the effect of immediate breast reconstruction with mastectomy in the local population.

Methods: The quasi-experimental, control study was conducted at the Shaukat Khanum Memorial Cancer Hospital and Research Centre, Lahore, Pakistan, and comprised patient data from April 2017 to December 2020, of breast cancer patients who underwent mastectomy with or without immediate breast reconstruction. Those who underwent mastectomy were labelled as control group A, while patients with immediate breast reconstruction were labelled as intervention group B.

Reverse total shoulder arthroplasty (rTSA) is an effective procedure to improve shoulder pain, range of motion, and function for a variety of conditions, including glenohumeral osteoarthritis and rotator cuff arthropathy. However, up to 22% of patients have persistent shoulder pain 12 to 24 months following rTSA, even in the absence of surgical complications. Currently, there are no widely accepted non-pharmacological treatments for persistent postoperative pain after rTSA.

  • Takotsubo syndrome (TTS) is a temporary heart condition characterized by ballooning of the heart's apex, often triggered by emotional or physical stress and associated with factors like sympathetic stimulation and certain medications.
  • Diagnosis includes checking specific heart activity patterns and using imaging techniques like angiography to rule out other heart issues.
  • A case report highlighted a 61-year-old woman with a history of TTS who experienced a recurrence during hospitalization, emphasizing the need for quick diagnosis and treatment to improve patient outcomes, especially in critical situations.

Several recent studies of carbon dots (CDs) synthesized by bottom-up methods under mild conditions have reported the presence of organic molecular fluorophores in CD dispersions. These fluorophores have a tendency to aggregate, and their properties strongly depend on whether they are present in the form of discrete molecules or aggregates. The aggregation becomes more prominent in the solid state, which motivates the study of the properties of the fluorophores associated with CDs in the solid state.

Mucosal Schwann cell hamartomas (MSCHs) are non-common noncancerous growths derived from Schwann cells in the peripheral nervous system, often found unexpectedly during routine colonoscopy examinations. These growths primarily occur in the colon, although they can also appear in the esophagus and are not linked to familial cancer syndromes. Diagnosis relies on specific histological characteristics and staining patterns.
