Disseminated vaccine-strain varicella as initial presentation of the acquired immunodeficiency syndrome: a case report and review of the literature.

Varicella-zoster virus (VZV) infections have declined in many industrialized countries due to vaccination with the attenuated Oka strain virus. Rare cases of severe, disseminated vaccine-strain VZV infection have occurred in the immunocompromised, although rarely in HIV-infected persons. We describe a man with previously-undiagnosed human immunodeficiency virus (HIV) infection who received VZV vaccination and subsequently presented to a combat hospital in Afghanistan with disseminated varicella, respiratory failure, and sepsis.

Advancing aspirin utilization: a review of clinical and systems-based interventions.

Underuse of clinical preventive services (CPS) such as low-dose aspirin therapy leads to tens of thousands of preventable deaths per year. The authors examined the current literature related to delivery of CPS and then provided the results to a preventive medicine expert panel who identified best practices to improve aspirin counseling and use. An exploratory literature search was conducted in PubMed using keywords associated with preventive health interventions.

Infected dermoid sinus tract mimicking recurrent sinusitis in an adolescent male.

Nasal dermoid sinus cysts are the most common congenital midline nasofrontal masses, but they can easily be confused with simple recurrent sinusitis upon clinical presentation. We report the case of a 14-year-old male who was initially diagnosed with frontal sinusitis and treated with antibiotics but, after multiple episodes of recurrence, was later sent for imaging. Computed tomography and magnetic resonance imaging showed an infected dermoid sinus tract.

Gnathodiaphyseal dysplasia.

Gnathodiaphyseal dysplasia (GDD) is an orthopedic condition involving cemento-osseous lesions of jaw bones, sclerosis, bowing of tubular bones, and overall bone fragility. This is a case report of GDD in which an 18-year-old female presented with a several-year history of bony abnormalities of the jaw and long bones. Radiographs demonstrated endosteal thickening, bowing of long bones, and abnormalities of the skull.

Carpal tunnel syndrome resulting from hydroxyapatite deposition.

Hydroxyapatite deposition disease (HADD) is a rare cause of carpal tunnel syndrome. This is a case report of a 55-year-old male who presented with symptoms of carpal tunnel syndrome and was subsequently found to have HADD. The diagnosis of HADD is made by the identification of poorly defined cloudy calcifications on radiography of the hand and wrist.
