Pullout strength of suture anchors in comparison with transosseous sutures for rotator cuff repair.

Suture anchors are increasingly gaining importance in rotator cuff surgery. This means they will be gradually replacing transosseous sutures. The purpose of this study was to compare the stability of transosseous sutures with different suture anchors with regard to their pullout strength depending on bone density.

Stress fractures presenting as tumours: a retrospective analysis of 22 cases.

Frequently, the imaging features of stress fractures may be misinterpreted as tumour-like lesions. The aim of this study was to analyse the quality of different examinations in detecting stress fractures mimicking tumour-like lesions in magnetic resonance imaging (MRI). We evaluated 22 cases which were referred to our department with the suspected diagnosis of bone tumours turning out to be stress fractures.

Selective inhibition of platelets by the GPIIb/IIIa receptor antagonist Tirofiban reduces leukocyte-endothelial cell interaction in murine antigen-induced arthritis.

Objective: Inflammation is associated with the invasion of leukocytes into affected tissues and with the up-regulation of platelet activation and adhesion. Assuming that leukocyte accumulation is linked to platelet aggregation, the aim of our study was to examine the effects of selective platelet inhibition by the glycoprotein (GP) IIb/IIIa receptor antagonist Tirofiban on the leukocyte-endothelial cell interaction.

Material And Methods: We used the model of antigen-induced arthritis (AiA) to induce inflammatory changes in the synovial microcirculation.

[The bending rod prosthesis: an experimental approach to metaphyseal hip arthroplasty].

The aim of our study was to develop a femoral component for total hip arthroplasty that would exclusively anchor in the metaphysis of the femoral neck. To forego trochanteric fixation, the load needs to be transferred to the metaphysis at as many points as possible. A computer simulation model suggested that an implant with a central cylinder and 16 rods aligned along a thread would be the preferable solution.

[Atypical course in Stickler syndrome].

We report about a 12-year-old boy with a Stickler syndrome. Because of osteonecrosis in the femoral head in MRI the initial diagnosis was dysplasia epiphysealis capitis femoris. The patient developed a progressive stiffness of the hip and a retinal detachment.

[The dislocated shoulder prosthesis--an avoidable disaster?].

Instability after shoulder arthroplasty is a common problem, even though complete dislocations are rare. A distinction can be made between vertical and horizontal instabilities. The most common type of vertical instability is superior migration of the humeral head caused by rotator cuff insufficiency; the shoulder prosthesis should be changed for an inverse prosthesis if the patient is symptomatic.

[Hip dislocation following THA].

In the event of a hip dislocation following THA analysis of its mechanism is the main priority. In addition, the time since the operation and the direction of the dislocation need to be taken into account. When the cause of the dislocation is analysed the formation of the neocapsule plays a part at least in the case of early dislocations (within the first 6 weeks after the operation).

In vivo interactions of platelets and leucocytes with the endothelium in murine antigen-induced arthritis: the role of P-selectin.

Objective: Platelets are thought to participate in the pathogenesis of chronic inflammatory diseases such as rheumatoid arthritis (RA). We showed recently an in vivo increase in platelet-endothelial cell interactions in mice with antigen-induced arthritis (AiA). The underlying mechanisms are not yet clear.

Platelet P-selectin is significantly involved in leukocyte-endothelial cell interaction in murine antigen-induced arthritis.

There is growing evidence that platelets play an important role in the development and maintenance of rheumatoid arthritis. Activation and adherence of platelets in the synovial microcirculation might be in part responsible for endothelial damage and activation of leukocytes. Recent findings show a direct influence of P-selectin on platelet- and leukocyte-endothelial cell interaction in mice with Antigen-induced Arthritis (AiA).

Endothelial iNOS versus platelet iNOS: responsibility for the platelet/leukocyte endothelial cell interaction in murine antigen induced arthritis in vivo.

Objective: Since an increase of platelet-endothelial cell interactions has been observed in mice with Antigen- induced-Arthritis (AiA) as well as an increase of NO expression, the aim of our study was to investigate in vivo the influence of NO, especially the platelet and endothelial inducible NO Synthase, on the platelet- and leukocyte endothelial cell interaction.

Material And Methods: C57/Bl6 mice and iNOS deficient mice were disposed in 6 groups (each=7). After induction of AiA, rolling and adherent fluorescence labelled platelets and leukocytes were investigated by intravital microscopy (IVM) on day 8 after AiA.

Proximal loading of the femur leads to low subsidence rates: first clinical results of the CR-stem.

Introduction: A new femoral stem was developed with a design that leads to compression of cancellous bone in the calcar region which results in proximal loading. The cross-sectional design of the implant provides rotational stability.

Materials And Methods: In the first clinical investigation ten patients underwent uncemented total hip arthroplasty between January 1999 and May 1999 using the CR-stem((R)) (Implantcast GmbH, Buxtehude, Germany).

In vitro long-term fatigue endurance of the secondary "Cement Injection Stem" hip prosthesis.

A secondary cementation hip stem (Cement Injection Stem; Aesculap, Tuttlingen, Germany) was designed to reduce the risk of fat embolism, and achieve precise implant position and high-quality cement mantle. A validated long-term in vitro simulation was carried out that replicated 24 years of activity of a very demanding patient. Inducible and permanent micromotions were monitored.

[Prognostic factors to assess the value of arthroscopy in gonarthrosis].

Aim: The value of athroscopy in osteoarthritis(oa) of the knee remains uncertain. Many studies claim to prove efficacy of arthroscopy in oa of the knee, while other authors report that there is no such effect. The purpose of this study was to identify factors to predict the outcome of arthroscopy in oa of the knee.

Navigated minimally invasive unicompartmental knee arthroplasty.

Unicompartmental knee arthroplasty (UKA) is an alternative procedure to high tibial osteotomy. This study assessed the procedure using computer navigation to improve implantation accuracy and presents early radiological results of a group of patients implanted with the univation UKA (B. Braun Aesculap, Tuttlingen, Germany) with navigation instrumentation and a minimally invasive approach.

Avascular necrosis of the femoral head: inter- and intraobserver variations of Ficat and ARCO classifications.

Diagnosis of avascular necrosis (AVN) of the femoral head depends on the combination of clinical symptoms and evaluation of radiographs and/or magnetic resonance imaging (MRI). To evaluate the evolution of AVN, the Ficat and the Association Research Circulation Osseous (ARCO) classification are commonly used to assess both imaging modalities. For comparison reasons, these classifications need to be reliable and reproducible to provide sufficient therapy options for the patient.

Timing of cervical spine stabilisation and outcome in patients with rheumatoid arthritis.

One complication of rheumatoid arthritis (RA) is the involvement of the cervical spine (CS). Although prophylactic stabilisation is recommended, the timing at which this should occur is poorly defined. The aim of our study was to evaluate the course of neurological symptoms in terms of the timing of surgery.

[Photoelastic stress analysis of human femurs before and after implantation of different models of femur neck prostheses].

Aim: The aim of this study was the detection of areas at risk at the proximal femur after implantation of different femur neck prostheses using the photoelastic stress analysis.

Methods: Twelve pairs of human femurs were used as examination material. The analysis of the stress pattern was done with a stepwise increasing load up to the quadruple of body weight before and after implantation of three models of femur neck prostheses which were implanted cementless.

Repair of osteochondral defects in the knee by resorbable bioimplants in a rabbit model.

Background: The intrinsic healing capacity of articular cartilage remains poor, despite various attempts that have been undertaken to treat cartilage defects. This study describes the experimental use of a double-layer bioimplant consisting of a bone-substitute layer and a cartilage-substitute layer.

Animals And Methods: In group A, 12 implants were placed into osteochondral defects of the load-bearing area of rabbit femoral condyles.

Long-term results of 58 hip cup revision arthroplasties using a threaded ring implant.

Introduction: There is still a discussion whether cementless hip cup revisions should be performed with a press fit cup or a threaded ring implant.

Materials And Methods: The results of 58 hip cup changes using the spherical, threaded ring "Munich" are presented. In 16 cases, the ring "Munich I" with a smooth surface and in 42 cases the ring "Munich II" with a corundum-blasted surface were implanted.

Of interest is how some countries have achieved a profound reduction of child injury mortality. Still little is reported on the impact of a combination of urban planning, social welfare development and safety measures. We therefore present trends in childhood (0-14 years) injury mortality in Sweden 1966-2001 and discuss the factors behind any reductions observed.

Percutaneous cryodenervation of lumbar facet joints: a prospective clinical trial.

Facet joint pain is an important aspect of degenerative lumbar spine disease, and radiofrequency medial branch neurotomy remains an established therapy, while cryodenervation has still been poorly examined. This study was undertaken to examine the effects of medial branch cryodenervation in the treatment of lumbar facet joint pain. This was a prospective clinical case series.

Classification of non-bacterial osteitis: retrospective study of clinical, immunological and genetic aspects in 89 patients.

Objective: To define non-bacterial osteitis (NBO) as a clinical entity possibly associated with autoimmune manifestations. Patients with sterile osteitis were analysed to develop diagnostic criteria.

Methods: A total of 89 patients with non-bacterial inflammatory bone lesions were observed for a median of 49 months.

Aggressive scapular chondroblastoma with secondary metastasis--a case report and review of literature.

Chondroblastoma is a benign bone tumor, accounting for approximately one percent of all benign bone tumors. It mostly occurs in typical locations such as long bones. Malignant transformation including metastasis has been described in only a few cases.
