Article Synopsis
  • Survival outcomes are generally poor for patients developing brain metastases, and there's a need to identify factors that may contribute to this condition.
  • The study focuses on examining the relationship between the use of Granulocyte-colony stimulating factor (G-CSF) and the occurrence of brain metastasis in patients with Stage IV breast cancer.
  • Findings indicate that G-CSF use did not significantly predict brain metastasis risk; however, pulmonary embolism (PE) and deep vein thrombosis (DVT) were found to be significant predictors of brain metastasis occurrence.

Article Synopsis
  • A 26-year-old woman developed a petechial rash and low platelet count (thrombocytopenia) two weeks after receiving the Moderna COVID-19 vaccine, leading to her being admitted to the emergency room.
  • During her hospital stay, her condition was complicated by elevated liver enzymes (transaminitis), which improved after treatment with intravenous steroids and immunoglobulin.
  • Extensive tests ruled out other possible causes for her symptoms, and although there was no definitive proof, the timing of her vaccination suggested that the vaccine might have triggered her health issues.

Acquired haemophilia A (AHA) is a rare and possibly fatal autoimmune disorder that is challenging to treat. Although a majority of cases are idiopathic, AHA can also be associated with an underlying malignancy, autoimmune disorder, pregnancy, infection or certain medications. The diagnosis and treatment of AHA require a specialist with both clinical and laboratory expertise.

Cytoreductive surgery (CRS) coupled with hyperthermic intraperitoneal chemotherapy (HIPEC) has become the standard of treatment for many cancers with peritoneal metastasis. Mitomycin-C (MMC), the most common chemotherapy utilized with HIPEC, is associated with neutropenia but the degree of hematologic toxicity is unclear when splenectomy is included as part of CRS with MMC. We present an interesting case of pancytopenia following treatment with HIPEC using MMC and comment on the possible role of splenectomy in exacerbating its cytotoxic effects.

Recent studies have shown that BRAF inhibitors, such as vemurafenib, are effective in inducing long periods of remission in relapsed hairy cell leukaemia. Acute pancreatitis is one of the rare complications that is reported with vemurafenib use. As severe pancreatitis can be life threatening, physicians should be vigilant of this side effect and promptly treat patients that develop clinical signs and symptoms while receiving vemurafenib.

Context: Gemcitabine is the backbone of systemic treatment of locally advanced and metastatic intrahepatic cholangiocarcinoma. In recent literature, gemcitabine has been linked to various pulmonary side effects.

Case Report: We report a case of an 82-year-old male who developed acute pulmonary hypertension after receiving one cycle of gemcitabine for metastatic cholangiocarcinoma.

The change of global-mean precipitation under global warming and interannual variability is predominantly controlled by the change of atmospheric longwave radiative cooling. Here we show that tightening of the ascending branch of the Hadley Circulation coupled with a decrease in tropical high cloud fraction is key in modulating precipitation response to surface warming. The magnitude of high cloud shrinkage is a primary contributor to the intermodel spread in the changes of tropical-mean outgoing longwave radiation (OLR) and global-mean precipitation per unit surface warming (dP/dT) for both interannual variability and global warming.
