Pancytopenia, cholestatic jaundice, and pulmonary hypertension are rare but serious complications of thyrotoxicosis. It is uncommonly seen in patients with hyperthyroidism and may result in treatment-related dilemmas as bone marrow suppression and drug-induced liver injury are known complications of anti-thyroid treatment.  We describe a case of Graves' disease presenting with all three manifestations of pancytopenia, cholestatic jaundice, and pulmonary hypertension during a relapse of thyrotoxicosis.

Autoimmunity associated with severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) has been well-described as the mechanism of development of thyroid dysfunction following Coronavirus Disease 19 (COVID-19) infection and SARS-CoV-2 vaccination. However, the occurrence of thyroid eye disease (TED) after SARS-CoV-2 vaccination is scarcely described. The postulated mechanisms include immune reactivation, molecular mimicry and the autoimmune/inflammatory syndrome induced by adjuvants (ASIA).

Thyroid abscess is rare but potentially fatal, whereas subclinical thyroiditis is a self-limiting condition that is more commonly seen. The clinical features of both diseases can be overlapping but the treatment approach differs. We report a case of a painful left thyroid nodule not responding to conventional therapy for thyroiditis or thyroid abscess.
