A 49-year-old patient, with a maternal history of arthritis, presented with an extra-capsular lump of the left knee. Following magnetic resonance imaging (MRI) scan and excision biopsy, the lump proved to be a benign ganglion cyst. The scan had also shown focal subchondral cyst formation and articular cartilage wear in the medial femoral condyle and patella, with no other abnormality.

Following an uneventful arthroscopic menisectomy of the right knee, a white circular skin lesion, 1 cm in diameter, was noted on the anterior left thigh of a 23-year-old patient. The overlying paper surgical drape had not ignited nor produced smoke. Close inspection revealed a minute perforation in the drape with slight discolouration.

A patient with hemophilia A, who developed factor VIII inhibitors and initially had funding for his total knee arthroplasty declined, is discussed. A total of £1 542 000 (US $2 474 603) was spent on medical treatment for recurrent hemarthroses over a 4-year period, while funding was sought. In comparison, the total cost for his knee arthroplasty was £542 858 (US $871 084) including perioperative recombinant clotting factors.

Use of a tourniquet for performing surgery in order to create a bloodless surgical field and reduce blood loss has been in use for many years. Tourniquets may fail perioperatively for various reasons, leading to ongoing bleeding. An important cause of tourniquet failure is calcification of the underlying artery.
