Successful two-stage revision of Lactobacillus infection of a total knee arthroplasty and literature review.

Background: Lactobacillus has been identified as the causative organism in only two total joint arthroplasties: one total hip arthroplasty (THA) and one total knee arthroplasty (TKA). The THA was a litigious case that was treated successfully with a one-stage revision, and the patient with the TKA ultimately required above-knee amputation. We present the first case report of a Lactobacillus infection of a TKA that was treated successfully with a two-stage revision TKA.

Some Historical Treatments should not be Forgotten: A Review of Cast Wedging and A Trick to Normalize Non-Standardized Digital X-rays.

Introduction: Cast wedging is a simple and reproducible method of manipulating a sub-optimally reduced fracture producing a correction and a final alignment that is amenable to definitive closed treatment. Multiple successful techniques have been previously described in the literature (opening wedge, closing wedge and combination).

Technical Note: We present a simple reproducible method of templating and executing a proper cast wedging technique using digital imaging systems that are not controlled for magnification with an illustrative case.

Concussions From 9 Youth Organized Sports: Results From NEISS Hospitals Over an 11-Year Time Frame, 2002-2012.

Background: Youth sports programs are extremely popular throughout the United States, with children starting formal sports participation as young as 4 years. This places children at greater risk for concussions and other trauma.

Purpose: To describe the epidemiology of concussions sustained during participation in 9 organized sports prior to participation in high school athletics.

Management of Cannulated Screw Failure and Recurrent SCFE Displacement - Case Report.

Introduction: SCFE occurs in 10 per 100,000 in some regions of the United States with the incidence continuing to increase. Percutaneous screw fixation is a well-accepted treatment for this disorder for over 20 years but management of complications is not well elucidated in the literature.

Case Report: We describe a case where a traumatic unstable SCFE that was initially treated with closed reduction and fixation with a single transphyseal screw went on to hardware failure with recurrence of the deformity.

Concussions From Youth Football: Results From NEISS Hospitals Over an 11-Year Time Frame, 2002-2012.

Background: Youth football programs across the United States represent an at-risk population of approximately 3.5 million athletes for sports-related concussions. The frequency of concussions in this population is not known.

Trabecular meshwork bradykinin receptors: mRNA levels, immunohistochemical visualization, signaling processes pharmacology, and linkage to IOP reduction.

Purpose: To localize mRNA and protein of bradykinin (BK) receptors, BK precursor polypeptide (kininogen) mRNA, and to study functional biochemical pharmacology of the signal transduction processes mediated by B2-receptors in isolated human trabecular meshwork (h-TM) cells. Intraocular pressure (IOP) lowering effects of 2 kinins were also investigated.

Methods: Previously documented procedures were utilized throughout these studies.

Unicompartmental knee replacement after high tibial osteotomy: Invalidating a contraindication.

The outcome of high tibial osteotomy (HTO) deteriorates with time, and additional procedures may be required. The aim of this study was to compare the clinical and radiological outcomes between unicompartmental knee replacement (UKR) and total knee replacement (TKR) after HTO as well as after primary UKR. A total of 63 patients (63 knees) were studied retrospectively and divided into three groups: UKR after HTO (group A; n = 22), TKR after HTO (group B; n = 18) and primary UKR (group C; n = 22).

Deficits of encoding in hypnosis: a result of altered state of awareness.

Because no studies have examined learning in hypnosis in an academic setting, the current study tested whether learning in hypnosis impacts test performance. Participants (N = 43) were randomly assigned into a hypnosis or a control group. Participants listened to an academic lecture, answered questions about their hypnotic depth, and completed a quiz based on the lecture.

Exon-level expression profiling of ocular tissues.

The normal gene expression profiles of the tissues in the eye are a valuable resource for considering genes likely to be involved with disease processes. We profiled gene expression in ten ocular tissues from human donor eyes using Affymetrix Human Exon 1.0 ST arrays.

Identification of PDE6D as a molecular target of anecortave acetate via a methotrexate-anchored yeast three-hybrid screen.

Glaucoma and age-related macular degeneration are ocular diseases targeted clinically by anecortave acetate (AA). AA and its deacetylated metabolite, anecortave desacetate (AdesA), are intraocular pressure (IOP)-lowering and angiostatic cortisenes devoid of glucocorticoid activity but with an unknown mechanism of action. We used a methotrexate-anchored yeast three-hybrid (Y3H) technology to search for binding targets for AA in human trabecular meshwork (TM) cells, the target cell type that controls IOP, a major risk factor in glaucoma.

Regional differences in practice patterns and associated outcomes for upper tract urothelial carcinoma in Canada.

Introduction: : We delineated Canadian regional differences in practice patterns in the management of upper tract urothelial carcinoma (UTUC) after nephroureterectomy and relate these to patient outcomes.

Methods: : A database was created with 1029 patients undergoing radical nephroureterectomy for UTUC between 1994 and 2009 at 10 Canadian centres. Demographic, clinical and pathological variables were collected from chart review.

Implant and limb alignment outcomes for conventional and navigated unicompartmental knee arthroplasty.

Accurate implant positioning and restoration of lower limb alignment are major requirements for successful long-term results in unicompartmental knee arthroplasty (UKA). Alignment accuracy was compared between navigated-UKA (nUKA) and conventional-UKA (cUKA) groups using a retrospective matched case-control study (n=129, 58 nUKA, 71 cUKA). Mechanical axis (MA), hip-knee-ankle angle (HKA°), coronal implant alignment, and tibial implant posterior slope were measured.

What do peer support workers do? A job description.

Background: The extant literature suggests that poorly defined job roles make it difficult for peer support workers to be successful, and hinder their integration into multi-disciplinary workplace teams. This article uses data gathered as part of a participatory evaluation of a peer support program at a psychiatric tertiary care facility to specify the work that peers do.

Methods: Data were gathered through interviews, focus groups, and activity logs and were analyzed using a modified grounded theory approach.

Recovery through the Lens of Cultural Diversity.

Topic: This Brief Report is based on a project entitled Recovery through the Lens of Cultural Diversity that sought to develop an enhanced model of recovery that accounts for culture.

Purpose: The purpose of the project was to increase the ability of mental health organizations in Toronto, Canada to design and deliver culturally relevant and responsive recovery-oriented services to the diverse communities they serve.

Sources Used: For the purposes of this report, sources used included an interdisciplinary and culturally diverse group of service providers; focus groups that included service users and family members; and a community forum.

Fracture through a Harris growth arrest line: a case report.

Fractures occur when bone is overloaded and mechanical failure occurs with the fracture propagating along the lines of least resistance. We report the case of a 6-year-old boy who sustained a nondisplaced distal third tibial shaft fracture through a Harris growth arrest line of increased osseous density due to low-energy blunt trauma. This case is used as an opportunity to review the literature on Harris growth arrest lines and discuss a fracture pattern that has not been previously described in the literature.

Epidemiology of cheerleading injuries presenting to NEISS hospitals from 2002 to 2007.

Background: Cheerleading has developed into a complex sport utilizing aspects of dance, gymnastics/tumbling, single- and multiple-partner stunts, and tosses in complex displays. Stunting and tosses elevates one or multiple participants into the air supported by teammates putting all at significant injury risk.

Methods: We reviewed all cheerleading injuries captured by the US Consumer Product Safety Commission National Electronic Injury Surveillance System from 2002 to 2007 that presented to US Emergency Departments and used the new narrative feature added in 2002 to better differentiate injury context and severity.

The use of a porcelain repair technique to improve the plane of occlusion of an existing restoration.

This article describes the use of an indirect porcelain repair technique to improve the occlusal relationship between a new fixed partial denture and an existing one. Porcelain repair techniques offer a conservative, minimally invasive, and cost-effective method of improving the opposing occlusion without replacing an existing restoration.

Exact infinite-time statistics of the Loschmidt echo for a quantum quench.

The equilibration dynamics of a closed quantum system is encoded in the long-time distribution function of generic observables. In this Letter we consider the Loschmidt echo generalized to finite temperature, and show that we can obtain an exact expression for its long-time distribution for a closed system described by a quantum XY chain following a sudden quench. In the thermodynamic limit the logarithm of the Loschmidt echo becomes normally distributed, whereas for small quenches in the opposite, quasicritical regime, the distribution function acquires a universal double-peaked form indicating poor equilibration.

An online learning approach to occlusion boundary detection.

We propose a novel online learning-based framework for occlusion boundary detection in video sequences. This approach does not require any prior training and instead "learns" occlusion boundaries by updating a set of weights for the online learning Hedge algorithm at each frame instance. Whereas previous training-based methods perform well only on data similar to the trained examples, the proposed method is well suited for any video sequence.

Purpose: A septic joint is a cartilage-threatening emergency requiring prompt treatment. The purpose of this study was to examine outcomes of septic arthritis of the metacarpophalangeal and interphalangeal joints.

Methods: We performed a retrospective review of patients diagnosed with joint infection between 1976 and 2008.

This study examined the relationship between proactive learning in hypnosis, post-hypnotic suggestion, and academic performance. Participants (N = 56) were randomly assigned to a control group or a treatment group. The treatment group was hypnotized and read a passage while in hypnosis.
