Many approaches to CMC arthroplasty have been described for treatment of advanced arthritis, yet there is no consensus on the "best" operation. Implantable hardware is increasingly utilized for metacarpal suspension, but few hardware-mediated complications have been documented. Background: Pyogenic flexor tenosynovitis (PFT) is frequently diagnosed by physical examination according to the Kanavel signs. This study proposes a modification of the Kanavel sign "tenderness over the course of the flexor sheath" by including palpation of the A1 pulley to increase specificity for diagnosis.

Osteomyelitis of the carpal bones is rare and usually occurs in the setting of penetrating wounds or immune compromise. A case of hardware-associated scaphoid osteomyelitis is reported in a 33-year-old man who underwent screw fixation of a scaphoid fracture more than 20 years prior to presentation. The patient was treated with operative debridement including a total scaphoidectomy as well as intravenous antibiotics.
