A 34-year-old female patient, with no comorbidities, presented with complaints of upper back pain across the shoulders, with altered sensation on the left side from trunk to lower limb, which was associated with reduced motor function and an acute symptom of urinary retention. On examination, there was reduced power in the left lower limb, reduced anal tone, a positive Babinski sign bilaterally, and reduced sensation in the perianal region. Serial magnetic resonance imaging (MRI) scans were conducted, where initially an upper thoracic lesion suggestive of an intramedullary cavernoma was found, and nearly a decade later, an adjacent extradural lesion causing cord compression was found incidentally through a surgical procedure.

A woman in her 70s presented with features of hyperandrogenism including clitoral enlargement and deepening of her voice. Biochemical investigations revealed raised plasma androgens and urinary androgen metabolites and imaging findings showed a highly F-18 fluorodeoxyglucose (FDG)-PET avid left adrenal tumour initially suspected to be a malignant adrenocortical carcinoma (ACC). She subsequently underwent an uncomplicated laparoscopic adrenalectomy where complete resection of her tumour was achieved.

Background: Congenitally corrected transposition of the great arteries (ccTGA) is a rare cardiac anomaly. The management strategy historically consisted of physiologic repair, leaving the morphologic right ventricle to support the systemic circulation. More recently, anatomic repair has been implemented to bring the left ventricle into the systemic circulation.

Purpose: The purpose of this work is to validate a simple and versatile integrated variable flip angle (VFA) method for mapping B in hyperpolarized MRI, which can be used to correct signal variations due to coil inhomogeneity.

Theory And Methods: Simulations were run to assess performance of the VFA B mapping method compared to the currently used constant flip angle (CFA) approach. Simulation results were used to inform the design of VFA sequences, validated in four volunteers for hyperpolarized xenon-129 imaging of the lungs and another four volunteers for hyperpolarized carbon-13 imaging of the human brain.

Background: Residual force enhancement (rFE), defined as increased isometric force following active lengthening compared to a fixed-end isometric contraction at the same muscle length and level of activation, is present across all scales of muscle. While rFE is always present at the cellular level, often rFE "non-responders" are observed during joint-level voluntary contractions.

Methods: We compared rFE between the joint level and single fiber level (vastus lateralis biopsies) in 16 young males.

Article Synopsis
  • Cushing's syndrome (CS) is rare in pregnancy and can be hard to identify because its symptoms are similar to those of normal pregnancy, as demonstrated in a case involving a 26-year-old woman.* -
  • The patient initially presented with gestational diabetes and pre-eclampsia; after delivery, ongoing hypertension and facial swelling led to her diagnosis of CS.* -
  • Successful treatment through laparoscopic adrenalectomy normalized her blood pressure, emphasizing the need for awareness of CS in pregnant women with gestational diabetes and hypertension to prevent complications.*

Objective: Throughout Europe, the interest in implementing robot-assisted minimally invasive direct coronary artery bypass (RA-MIDCAB) has been growing. However, concerns about additional costs have emerged concurrently. In this analysis, we aim to provide a comparison of the cumulative perioperative costs of RA-MIDCAB, on-pump coronary artery bypass grafting (CABG), and off-pump CABG (OPCAB).

Background: The difficulty of suturing perfect anastomoses in limited-access conditions prevents the transition of traditional coronary artery bypass grafting (CABG) to sternal-sparing approaches, even in the robotic era. Automated coronary anastomotic connector technologies may address these difficulties, but to date, none have achieved broad adoption. Besides versatility, ease-of-use and cost-effectiveness, the key performance parameter of such technology is anastomotic patency.

A one-pot microwave assisted telescopic approach is reported for the chemo-selective synthesis of substituted 1,3-thiazetidines using readily available 2-aminopyridines/pyrazines/pyrimidine, substituted isothiocyanates and 1,2-dihalomethanes. The procedure involves thiourea formation from 2-aminopyridines/pyrazines/pyrimidine with the substituted isothiocyanates followed by a base catalysed nucleophilic attack of the CS bond on the 1,2-dihalomethane. Subsequently, a cyclization reaction occurs to yield substituted 1,3-thiazetidines.

A series of mixed-anion Fe(NHtrz)(BF)(SiF) spin crossover complexes is obtained modifying the reaction time but also using an increase amount of tetraethyl orthosilicate as the source for the production and the incorporation of SiF competing with the BF anions present in the mother solution. The increase of the SiF anion inclusion to the detriment of the BF counterpart induces a shift of the temperature transition toward high temperatures leading to interesting bistability properties around room temperature with spanning from 300 K to 325 K. Moreover, the implementation of a solid-liquid post synthetic modification approach from the Fe(NHtrz)(BF) parent complex with identical TEOS proportions and under certain experimental conditions lead systematically to the same Fe(NHtrz)(BF)(SiF) composition.

Electromagnetic methods for non-destructive evaluation (NDE) are presented, with which sheet metal components can be identified and their material properties can be characterized. The latter is possible with 3MA, the Micromagnetic Multiparametric Microstructure and stress Analyser. This is a combination of several micromagnetic NDE methods that make it possible to analyse the microstructure in a ferromagnetic material and to determine quantitative values of the mechanical material properties or the stress state.

Lynch syndrome is an autosomal dominant disorder that usually results from a pathogenic germline variant in one of four genes (MSH2, MSH6, MLH1, PMS2) involved in DNA mismatch repair. Carriers of such variants are at risk of developing numerous cancers during adulthood. Here we report on a family suspected of having Lynch syndrome due to a history of endometrial adenocarcinoma, ovarian clear cell carcinoma, and adenocarcinoma of the duodenum in whom we identified a germline 29 nucleotide in-frame inversion in exon 3 of MSH2.
