Changes in ambulatory surgery utilization in the metropolitan area of Syracuse, New York 1983-88 were identified. Total ambulatory surgery utilization rose from 12,479 to 24, 643 procedures. Approximately 69 percent of the increase was generated by freestanding surgery centers.

In the absence of controlled clinical trials, the diffusion of magnetic resonance imaging (MRI) has been driven by market forces and the perceived benefits of this technology. To date, all projective needs for MRI use are based on a consensus impression of a medical panel on the role of MRI for DRG or International Classification of Diseases, Ninth Revision, codes. Since an impression of future utilization is not particularly precise, the focus of The New York State MRI Demonstration Project, which approved the acquisition of MRIs at 14 medical centers in 1983, was to determine the actual use of MRI in a medical setting.

The evaluation and reimbursement of hospital use by means of diagnosis-related groups (DRGs) may have a major impact on the utilization of regional neonatal care. Medicare has already implemented the DRG system and other payors, including Medicaid, Blue Cross, and commercial insurance, are also considering adopting it. Under this approach, neonates are assigned to one of seven DRGs, each of which is reimbursed at a relatively fixed rate.

We conducted a study to determine the utilization level for ambulatory surgery and the proportions of specific procedures which could be performed in certain ambulatory settings in one community, Syracuse, New York, which has used this service extensively. It was estimated that, in 1983, 26.9 surgery cases per 1,000 population in Syracuse were performed in ambulatory settings.
