Purpose Of Review: Effective pain management in cardiac surgery presents as a continuous challenge related to the intensity of postoperative pain and reliance on opioid therapy. The dependance of opioid-based therapies is concerning, as these therapies carry risk future addiction and potential severe side effects. The transversus thoracic plane block (TTPB) has emerged as a promising regional anesthesia technique that blocks the anterior branches of the intercostal nerves in the chest wall, potentially providing improved analgesia for cardiac surgery patients.

Objective: To compare the efficacy and safety of administering six doses of intrapleural streptokinase (SK) versus the conventional three doses, in children with empyema.

Study Design: In this open label, placebo-controlled, randomized trial, we enrolled 53 children with empyema, who received three doses of intrapleural SK. Thereafter, those without clinical improvement (n = 34) and those showing clinical improvement but having persistent pleural fluid width > 10 mm on chest ultrasonography (n = 13), were randomized to receive three additional doses of SK, or three doses of placebo (normal saline).

A 50-year-old woman with a history of adult granulosa cell tumor (AGCT) of the right ovary was under follow-up after undergoing several surgeries, including a total abdominal hysterectomy with bilateral salpingo-oophorectomy. She was initially diagnosed eight years ago and remained disease-free for 52 months. However, she later experienced a recurrence, indicated by elevated inhibin B levels (58 ng/mL) and the presence of peritoneal soft tissue tumors.
