Article Synopsis
  • This study focuses on the importance of predicting the time to renal replacement therapy (RRT) for patients at a high risk of end-stage kidney disease.
  • Researchers developed and tested machine learning models using data from chronic kidney disease (CKD) patients at Oita University Hospital, comparing the accuracy of these models to traditional methods based on the rate of estimated glomerular filtration rate (eGFR) decline.
  • The machine learning approach demonstrated moderate predictive accuracy (R of 0.60), significantly outperforming the conventional method, which had an extremely low accuracy (R of -17.1), highlighting the potential for machine learning to improve CKD treatment predictions.

Article Synopsis
  • Morphine is commonly used to treat dyspnea, but other opioids like oxycodone and fentanyl are also being utilized, leading to the practice of opioid switching, which lacks verified effectiveness for this symptom.
  • A study took place at Komaki City Hospital analyzing 57 patients who switched opioids for dyspnea relief, finding that 36.8% experienced effective results after one week.
  • Factors such as age, prognosis, and presence of cachexia were significant predictors of successful opioid switching for dyspnea, with no serious adverse effects reported.

Background: The appropriate timing of introducing the home-based nursing service to patients with advanced cancer has not been clarified. This study conducted a retrospective cohort study to investigate the associations of the early utilization of the home-based nursing service during chemotherapy with the place of care immediately after the last chemotherapy treatment in patients with advanced cancer.

Methods: Among the patients referred to the palliative care team of the University of Tsukuba Hospital between January 2018 and December 2019, patients with advanced cancer undergoing chemotherapy or before the start of chemotherapy at referral were included.

We report two cases of idiopathic multicentric Castleman disease (iMCD) with nephrotic syndrome (NS) treated with tocilizumab. Case 1 was a 58-year-old man diagnosed with iMCD prior to the onset of NS. Renal biopsy revealed membranous nephropathy, which was considered to be secondary membranous nephropathy associated with iMCD.

To determine factors related to the effectiveness of red blood cell transfusions in terminally ill cancer patients, we conducted a retrospective cohort study. We studied 27 terminally ill cancer patients who were transfused with red blood cells at our hospitalbetween June 2015 and September 2017. Patients who responded to the treatment("responders"; 16 cases[59%]) were compared with those who did not("non-responders"; 11 cases[40%]).
