Introduction: A concave iris configuration is a characteristic feature of pigment dispersion syndrome (PDS). Therapeutic strategies to modify this configuration may be considered as first-line options for the treatment of ocular hypertension in PDS patients.

Case Description: A 37-year-old male patient with myopic anisometropia presented bilateral PDS with concave iris, Krukenberg's spindle and iris transillumination defect in both eyes.

Importance: Hyponatremia treatment guidelines recommend limiting the correction of severe hyponatremia during the first 24 hours to prevent osmotic demyelination syndrome (ODS). Recent evidence suggests that slower rates of correction are associated with increased mortality.

: To describe the pseudochamber-protected keratoplasty (PPK) procedure with the new Endo-K Pro implant technique and report the clinical outcomes in patients at high risk for penetrating keratoplasty (PKP). : This case series study included patients who required a PKP and had a high risk for corneal transplant failure. All cases underwent the PPK procedure with simultaneous Endo-K Pro implantation and had a minimum follow-up of 12 months.
