Background: Alzheimer's disease and related dementias (ADRD) prevalence varies geographically in the US. The Appalachian region has lower educational attainment and health care access barriers compared to non-Appalachian regions. The objective of this proposal is to assess whether the geographic variation of ADRD in Central Appalachia is explained by county-level sociodemographic factors or access to care.

Rural and Appalachian areas are characterized by limited access to primary and specialty health care in comparison to their urban and non-Appalachian counterparts. Better access to telehealth can improve detection of ADRD in remote regions but it highly relies on availability of broadband services.

The heterodimeric Rab3GAP complex is a guanine nucleotide exchange factor (GEF) for the Rab18 GTPase that regulates lipid droplet metabolism, ER-to-Golgi trafficking, secretion, and autophagy. Why both subunits of Rab3GAP are required for Rab18 GEF activity and the molecular basis of how Rab3GAP engages and activates its cognate substrate are unknown. Here we show that human Rab3GAP is conformationally flexible and potentially autoinhibited by the C-terminal domain of its Rab3GAP2 subunit.

Introduction: Anastomotic leak (AL) represents a significant complication following gastrointestinal (GI) surgery, contributing to increased morbidity and mortality. pH monitoring has emerged as a potential diagnostic tool for the early detection of AL, but its effectiveness and clinical utility remain to be fully elucidated. This review aims to summarise the evidence regarding perianastomotic pH monitoring for AL detection.
