Objectives: The prognosis of bone and joint infections (BJI) caused by Gram-negative bacilli (GNB) worsens significantly in the face of fluoroquinolone-resistance. In this setting, scarce pre-clinical and clinical reports suggest that intravenous beta-lactams plus colistin may improve outcome. Our aim was to assess the efficacy and safety of this treatment in a well-characterized prospective cohort.

Article Synopsis
  • Prosthetic joint infections (PJIs) caused by Candida species are serious complications following joint replacement surgeries, with a study reviewing 269 cases between 2010 and 2021 to assess treatment outcomes.
  • The majority of infections occurred in older patients (average age 73), primarily in hips and knees, and most cases involved additional bacterial infections; roughly 58% achieved a cure at the two-year follow-up.
  • Treatment effectiveness varied significantly depending on the surgical method used, with poorer outcomes linked to the debridement, antibiotics, and implant retention (DAIR) approach and patients older than 70, while infections from Candida parapsilosis tended to have better outcomes.

Article Synopsis
  • - The study investigates rare cases of tuberculous prosthetic joint infections (PJIs) by analyzing newly identified instances and existing literature to find factors affecting patient outcomes.
  • - Researchers collected data from unreported cases across four countries and reviewed 155 published cases, noting that the majority of patients (76 years old on average) had hip infections and many experienced co-infections.
  • - Results indicated that while antibiotic therapy led to successful cures, neither surgical intervention nor treatment duration significantly influenced recovery, highlighting the chronic nature of this disease.

Prosthetic joint infections are considered difficult to treat they needing aggressive surgery and long antimicrobial treatments. However, the exact duration of these therapies has been established empirically. In the last years, several studies have explored the possibility of reducing the length of treatment in this setting, with conflicting results.

Background: Rheumatoid arthritis (RA) is a risk factor for periprosthetic joint infection (PJI) after total joint arthroplasty (TJA). The purpose of this study was to perform a systematic review comparing the failure rates of debridement, antibiotics, and implant retention (DAIR), one-stage exchange arthroplasty/revision (OSR), and 2-stage exchange arthroplasty/revision (TSR) for RA patients with PJI and identify risk factors in the RA population associated with increased treatment failure rate.

Methods: PubMed, Ovid MEDLINE, and Ovid Embase databases were screened with the terms "rheumatoid arthritis," "total joint arthroplasty," "prosthetic joint infection," and "treatment for PJI" on August 29, 2021.
