Background Randomized clinical trials have demonstrated that endovascular thrombectomy reduces functional disability in patients with large ischemic stroke; arterial collateral status might be used to select these patients for endovascular thrombectomy. Purpose To investigate whether arterial collateral status modifies the treatment effect of endovascular thrombectomy in patients with large ischemic stroke. Materials and Methods The Efficacy and Safety of Thrombectomy in Stroke with Extended Lesion and Extended Time Window (TENSION) trial was a prospective, multicenter, randomized study investigating participants with acute large ischemic stroke due to anterior circulation large-vessel occlusion.

Objectives: To evaluate the effectiveness of 18 different host biomarkers in differentiating bacterial from non-bacterial acute febrile illness (AFI) in resource-limited settings, specifically in Brazil, Malawi and Gabon.

Design: Multinational, cross-sectional study.

Setting: The study was carried out across multiple primary healthcare facilities, including urban and rural settings, with a total of three participating centres.

Article Synopsis
  • - The TENSION trial evaluated the long-term safety and effectiveness of endovascular thrombectomy for patients with ischemic stroke and large infarcts, demonstrating favorable outcomes at 90 days, with a follow-up extending to 12 months.
  • - Conducted across various hospitals in Europe and Canada, the trial involved patients aged 18 and older with acute ischemic strokes caused by large vessel occlusions, comparing results between those receiving thrombectomy plus medical care versus medical care alone.
  • - Out of 253 enrolled patients, results indicated significant insights into functional outcomes, quality of life, and mental health aspects like anxiety and depression, although the trial concluded early due to promising efficacy in the treatment group.

Article Synopsis
  • - Diaphragmatic myoclonus is a rare condition involving involuntary contractions of the diaphragm and can have serious implications for those affected.
  • - A case study is presented of a 61-year-old patient whose condition was linked to a bleeding cavernoma in the Guillan-Mollaret triangle, highlighting the specific challenges faced in treatment.
  • - The patient successfully received botulinum toxin injections into the abdominal muscle after oral treatments failed, emphasizing the need for accurate diagnosis and effective interventions for severe cases of this condition.

Vascular dementia (VaD) is a white matter ischemic disease and the second-leading cause of dementia, with no direct therapy. Within the lesion site, cell-cell interactions dictate the trajectory towards disease progression or repair. To elucidate the underlying intercellular signaling pathways, a VaD mouse model was developed for transcriptomic and functional studies.
