Importance: Transition to adult care is a challenging and complex process for youth and emerging adults with chronic health and/or mental health conditions. Patient navigation has been proposed to improve care during transition, but previous studies have used single disease cohorts with a nonrandomized design.
Objective: To compare the effectiveness of a patient navigator service to reduce emergency department (ED) use among adolescents and emerging adults with chronic health and/or mental health conditions undergoing transition to adult-oriented health care.
The family Hypoxylaceae has a cosmopolitan distribution with greater diversity in tropical regions, its growth habit is saprotrophic, endophytic and potentially phytopathogenic. From the revision of herbarium specimens and field collections from the Yucatan Peninsula, two new species were described: , characterized by having fusiform spores and which is distinguished by having stromata vinaceous and dark brown KOH-extractable pigments. The species are described based on morphological characters and Bayesian Inference analyses of four molecular markers (ITS, LSU, RPB2 and TUB2).

A variety of evaluation approaches have been used to assess the initiative.
