The introduction of hybrid diagnosis-related groups (DRG) presents new challenges for healthcare providers and health insurances. The same applied in 2023 to the institute designated by the Federal Ministry of Health (BMG) to extract medical procedures and calculate remuneration levels for the first hybrid DRGs. A responsible calculation methodology and a realistic data basis are required as the result of the calculation can lead to controversy, even to a splitting among specialist groups and constructs.

The concept of an active dosimetry system for pulsed radiation dose rate measurements is presented. Real-time distinction of pulsed and non-pulsed radiation contributions is based on the time structure of a single interaction. A fast tissue equivalent plastic scintillator is exploited to minimize the pile-up effect influence on absorbed energy measurements.

In the course of this work, a dosemeter for measurement of ambient dose equivalent H*(10) based on the BeOSL dosimetry system was designed. Calculations of the energy response of the two-element BeOSL personal dosemeter in terms of H*(10) showed that the response does not fulfil the requirements of IEC 62387 Especially, the response for photon energies of 60-100 keV is too low. It is possible to correct this under-response using a BeOSL two-element dosemeter, a linear algorithm and an additional cover for the dosemeter.

The Individual Monitoring Service of the Helmholtz Zentrum München is currently using the BeOSL dosimetry system for monitoring ∼15 000 persons per month. This dosimetry system has a modular structure and represents a complete new concept on handling dosemeters in a large-scale dosimetry service. It is based on optically stimulated luminescence dosemeters made of beryllium oxide.

A new official albedo dosemeter based on thermoluminescent detectors has been introduced in 2015 by the individual monitoring service of the Helmholtz Zentrum München for monitoring persons who are exposed occupationally against photon and neutron radiation. To enhance the sensitivity for fast neutrons, a new badge with an enlarged albedo window has been developed at TU Dresden. The properties of the new albedo dosemeter are discussed, and the results of official intercomparisons and field calibrations are shown.
