The authors report a case of xanthogranulomatous pyelonephritis complicated by renocolic fistula. This is a rare complication. The patient was treated by right nephrectomy and closure of the fistula performed by manually assisted laparoscopic surgery.

Computed tomography and arteriography showed that these varices were situated on the convex surface of the kidney, in front of its anterior surface, and drained venous blood derived from the renal parenchyma. These varices were probably secondary to undiagnosed renal vein thrombosis.
