Optimal surgical timing for ear reconstruction with autologous cartilage: Analysis of the computed tomography scan characteristics of the ribs.

Background: The approach to constructing the cartilage framework for ear reconstruction is sufficiently established. However, there is still no consensus about the age of initiation of surgical treatment. This study aims to assess the development and growth of the costal cartilage to determine the best age to perform ear reconstruction surgery.

12-month follow-up after successful infliximab therapy in pediatric crohn disease.

Aim: Infliximab (IFX) therapy is highly efficacious for the induction and maintenance of remission in pediatric Crohn disease (CD). However, to date it is unclear how long patients should be given IFX. Given the increasing safety concerns about the concomitant and prolonged use of IFX and azathioprine in CD, we wanted to address the clinical outcome in pediatric CD patients who responded to IFX medication, once IFX was stopped.

Profound hypoxemia during treatment of low cardiac output after cardiopulmonary bypass.

Purpose: To illustrate the multiple causes of hypoxemia to be considered following cardiopulmonary bypass and how therapy given to improve oxygen delivery may have contributed to a decrease in arterial oxygen saturation to life-threatening levels.

Clinical Features: A 61 yr old man with severe mitral regurgitation and chronic obstructive lung disease underwent surgery for mitral valve repair. A pulmonary artery catheter with the capacity to measure cardiac output and mixed venous oxygen saturation (SvO2) continuously was used.

Purpose: There have been great improvements in the management of patients with heart disease over the past 50 yr much of which has been due to the development of surgical procedures for the correction of acquired and congenital cardiac abnormalities. A great deal has been written about the surgeons and the innovative procedures they developed. They were undoubtedly courageous, imaginative, knowledgeable and skillful.
