The anti-hypertensive effect of captopril 50 mg plus hydrochlorothiazide 25 mg, given once or twice daily, was investigated by 24 h monitoring in hypertensive patients in a controlled, randomized, double-blind, cross-over trial. Ten patients with blood pressure (BP) greater than 160 and/or 95 mm Hg after a 15 day placebo period, were randomly assigned to one of two treatments, in which they either received the combination once daily in the first month, twice daily in the second and once daily in the third (Treatment Group A) or twice daily in the first month, once daily in the second month and twice daily in the third month (Treatment Group B). The anti-hypertensive effect of the drug combination was assessed by 24 h monitoring using a Dinamap 845 to establish mean BP levels and circadian curves.

The results of a survey carried out in Marseilles between 1972 and 1979 are reported. A common protocol was used to compare results with two other surveys performed conjointly in Paris and Brussels. The protocol was designed with special emphasis on the psychological factors with respect to the risk factors for ischaemic heart disease (IHD).

The authors address the two following questions: 1) Is detection of glycoregulation disorders possible from data yielded by routine annual examination (7,923 subjects)? 2) Do further investigations, carried out in a subdivision of subjects selected according to the risk of cardiovascular disease (n = 1,232), show a difference in the distribution of risk factors and risk indicators between patients and "healthy" individuals? Analysis of the results of this transversal study demonstrates a fact that is well-known concerning diabetes: even mild hyperglycemia (greater than or equal to 6 Mmol/l) is significantly correlated to age, excess weight and high systolic blood pressure. In both male and female subjects, there is a strong correlation between mean blood glucose concentration and the other biologic parameters that were studied (blood pressure, serum cholesterol, LDL/HDL cholesterol). Conversely, no correlation was found with smoking habits or with the intake of various dietary components.
