Article Synopsis
  • IPF development involves a two-hit process: an initial vulnerability of the lung's epithelial cells followed by a second injury that triggers fibrotic changes.
  • Genetic factors play a crucial role in the first hit, while ongoing damage disrupts normal cell function and activates fibroblasts, leading to lung fibrosis.
  • Understanding IPF in terms of its stages may lead to new treatments that target these underlying causes, aiming for a shift from palliative care to potential cures for patients.

Objective: Stereotactic neuromodulation, such as deep brain stimulation (DBS) and responsive neurostimulation (RNS), have emerged as some of the more promising means for managing drug-resistant epilepsy. This study serves as a comprehensive analysis of DBS of the anterior nucleus of the thalamus (ANT), centromedian thalamic nucleus (CMT), and hippocampus and RNS for seizure reduction in adult intractable epilepsy.

Methods: Following PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines, a systematic review was conducted of PubMed, Cochrane Library, and Embase databases from January 2000 to January 2024 to objectively assess the effectiveness of the various neuromodulation modalities on seizure reduction.

Background: Allogeneic bone marrow transplantation (BMT) may be a curative treatment for patients with rheumatoid arthritis (RA), but it has serious risks, including death. It is uncertain whether patients would accept the risks and benefits of BMT and participate in clinical trials. We conducted a discrete choice experiment (DCE) to quantify risk tolerance and benefit-risk trade-offs to inform the design of a clinical trial for BMT.

Objectives: To evaluate patient preferences for decision-making role in the management of intraductal papillary mucinous neoplasms (IPMNs) of the pancreas and to identify individual characteristics associated with those preferences.

Background: Management of IPMNs is rooted in uncertainty with current guidelines failing to incorporate patients' preferences and values.

Methods: A representative sample of participants aged 40-70 were recruited to evaluate a clinical vignette where they were given the option to undergo surveillance or surgical resection of their IPMN.
