The authors analyse the outcomes of surgical management in 400 patients with the Wolff-Parkinson-White syndrome who had been operated on in the All-Union Centre for Surgical treatment of Severe Cardiac Arrhythmias and Electrocardiostimulation. Unique surgical procedures were applied, which made it possible to perform cryodestruction of accessory atrioventricular pathways, without resorting to an extracorporeal circulation and without opening heart cavities. The procedures are demonstrated to be not inferior to operations using extracorporeal circulation.

The first clinical experience of the successful application of electrostimulation of the atrial septum is described. Sickle-shaped electrodes were used for this purpose. Electrostimulation of the atrial septum was medically indicated in all cases.

Re-entry tachycardia was found in 182 patients of a total of 275 subjected to electrophysiological investigation of heart function; in 26 of those, a radio-frequency atrial electrostimulation system was implanted. Early postoperative complications were noted in 3 patients: subcutaneous hematoma in 2, and skin decubitus in 1. Long-term follow-up (as long as 7 years) confirmed the efficiency of atrial radio-frequency pacing.
