Article Synopsis
  • Patients undergoing pancreaticoduodenectomy for distal cholangiocarcinoma (dCCA) have a high recurrence rate, with 65% developing recurrence mostly within three years post-surgery.
  • The study identified common recurrence patterns, including local, distant, and mixed types, with primary sites being the pancreatic bed, liver, and lungs.
  • Key predictive factors for recurrence included cancer stage, type of surgical resection, and various histological features, helping inform potential follow-up treatments or strategies.

Background: Bile duct injury (BDI) causes significant sequelae for the patient in terms of morbidity, mortality, and long-term quality of life, and should be managed in centers with expertise. Anatomical variants may contribute to a higher risk of BDI during cholecystectomy.

Aims: To report a case of bile duct injury in a patient with situs inversus totalis.

Multiple hepatocellular carcinoma (HCC) staging systems have been proposed and used clinically over time. These may consider clinical, pathological, radiological, or treatment response factors, depending on the model. Given the heterogeneity of HCC treatment in its different stages and the validation of the systems in different populations, they are not universal.

Introduction: Pancreas is considered one of the organs most frequently affected by recurrence after nephrectomy secondary to renal cell carcinoma reporting an incidence of 20%, 85% of these occur within the first 3 years.

Objective: The objective of the study is to evaluate overall survival and disease-free survival in patients with renal cancer and pancreatic metastases who underwent surgical treatment.

Methods: A retrospective cross-sectional study of patients with histological diagnosis of renal cancer associated with pancreatic metastasis was performed and included those treated by pancreatoduodenectomy or distal pancreatectomy during the period 1987-2020.

Background: An increasing number of hepatic artery infusion (HAI) programs have been established worldwide. Practice patterns for this complex therapy across these programs have not been reported. This survey aimed to identify current practice patterns in HAI therapy with the long-term goal of defining best practices and performing prospective studies.

Background: The Mexican Hepato-Pancreato-Biliary Association (AMHPB) conducted a survey, aiming to gather experience and opinions of HPB surgeons about HPB surgery in the 2020 COVID-19 pandemic year.

Methods: An online survey was conducted (33 items: demographics, patient referral, COVID-19 screening and limitations, hospital occupancy and surgical practice) to AMHPB members and attendees of the previous meetings of the Society through a self-administered questionnaire. Answers were excluded if respondents were not from Latin America.

Pancreatic cancer is a pathology with a high mortality rate since it is detected at advanced stages, so the search for early-stage diagnostic biomarkers is essential. Liquid biopsies are currently being explored for this purpose and educated platelets are a good candidate, since they are known to present a bidirectional interaction with tumor cells. In this work, we analyzed the effects of platelets on cancer cells' viability, as determined by MTT, migration using transwell assays, clonogenicity in soft agar and stemness by dilution assays and stem markers' expression.

Background: Widespread implementation of HAI pump chemotherapy has been limited by logistic and feasibility concerns. Recent studies demonstrating excellent outcomes have fueled renewed enthusiasm and multiple new programs have emerged. This survey aims to identify barriers critical to establish a successful HAI program.

Liver function tests help in the follow-up of postoperative patients with iatrogenic bile duct injury. There is not clear evidence regarding their predictive role on anastomosis dysfunction. We describe our experience with postoperative liver function tests and a predictive model of long-term patency after repair.

Objective: The aim of this study is to assess the perceptions of the impact of health-care disruption due to COVID-19 on the academic training and skills of surgical trainees.

Material And Methods: We developed a 32-question survey assessing the clinical and surgical impact of COVID-19 on surgical training programs and proposals to compensate for the decrease in surgical education. We got 453 responses of surgical trainees in Mexico City.

Objective: To analyze the perioperative and long-term outcomes of patients undergoing LT due to BDI in a tertiary care center.

Background: BDI is associated with significant morbidity and long-term impact on quality of life. LT represents the only possibility of a cure in patients with BDI who develop SBC.

In the past three decades, several technologies designed for other purposes, have been applied in surgery to provide more precision to the surgical procedures and better outcomes. In surgery, innovation requires evidence before widespread implementation of novelties and a continuous quality improvement process to assess benefits and risks. Robotics in surgery has been widely implemented, but in some cases, there are many doubts regarding its clinical benefit and cost utility.

Article Synopsis
  • The study aimed to determine the impact of neoadjuvant therapy (NAT) on the rates of complete tumor resection (R0) and long-term survival outcomes in patients with pancreatic ductal adenocarcinoma undergoing surgery with vein resection.
  • Overall, results from 1192 patients showed that those who received NAT had significantly higher rates of R0 resection (57%) and better survival rates after 1, 3, and 5 years compared to those who did not receive NAT.
  • The findings suggest that NAT should be considered routinely for patients with pancreatic ductal adenocarcinoma scheduled for surgery involving venous reconstruction.

Background: Bile duct injury (BDI) is accompanied by significant morbidity and long-term impact in quality of life. Subtotal cholecystectomy (STC) is an alternative to prevent this outcome but is associated with other complications. The aim of this work is to demonstrate that BDI associated morbidity exceeds STC associated morbidity, underscoring STC as a reasonable bail out strategy.

Unlabelled: Idiopathic acute recurrent pancreatitis (IARP) is defined as at least two episodes of acute pancreatitis with the complete or near-complete resolution of symptoms and signs of pancreatitis between episodes, without an identified cause. There is a paucity of information about the usefulness of endoscopic ultrasound (EUS) in IARP.

Objectives: To determine the diagnostic yield of EUS in IARP.

Background: Several guidelines have put forward recommendations about the perioperative process of cholecystectomy. Despite the recommendations, controversy remains concerning several topics, especially in low- and middle-income countries. The aim of this study was to develop uniform recommendations for perioperative practices in cholecystectomy in Mexico to standardize this process and save public health system resources.

Objective: The aim of this study was to establish clinically relevant outcome benchmark values using criteria for pancreatoduodenectomy (PD) with portomesenteric venous resection (PVR) from a low-risk cohort managed in high-volume centers.

Summary Background Data: PD with PVR is regarded as the standard of care in patients with cancer involvement of the portomesenteric venous axis. There are, however, no benchmark outcome indicators for this population which hampers comparisons of patients undergoing PD with and without PVR resection.

Background: An international group proposed a standardized terminology to report outcomes after bile duct repair. Data on this surgical complication vary depending on the center and country where patients are treated. The aim of this work is to show disparities in the care process of bile duct injury between patients from two different income-level countries, using a standard terminology of outcomes and clinical reporting.
