During the time-course of cognitive decline, Behavioral and Psychological Symptoms of Dementia (BPSD) may arise, negatively impacting the outcomes. The aim of this single center, longitudinal study was to evaluate the correlation between frailty and BPSD in a population of older patients with dementia. BPSD were classified into three clusters: "mood/apathy" (depression, apathy, sleep disturbances, appetite disturbances), "psychosis" (delusions, hallucinations, and anxiety), and "hyperactivity" (agitation, elation, motor aberrant behavior, irritability, disinhibition).

Article Synopsis
  • Hospital-at-home (HaH) services, like the "Gruppo di Intervento Rapido Ospedale-Territorio" (GIROT) in Florence, offer an alternative to traditional hospital care for older adults, potentially minimizing hospital admissions and complications.
  • A study of 391 patients (average age 88.4 years) revealed high mortality rates over 6 months (53.8%) and hospitalizations (37.9%), with predictors for 6-month mortality including age and severe disability.
  • The GIROT model demonstrates high patient satisfaction and suggests the need for tailored care plans for older patients at risk of complications.

Background: Frailty detection with comprehensive geriatric assessment (CGA) is of pivotal importance in older patients with cancer to avoid over- or under-treatment and to detect those at increased risk for poor outcomes. Several tools have been developed to capture the complexity of frailty, but only a few were explicitly conceived for older adults with cancer. The study aimed at developing and validating a multidimensional, easy-to-use diagnostic tool for early-risk stratification in patients with cancer, called the Multidimensional Oncological Frailty Scale (MOFS).
