Implementation of an innovative model of community nursing for older adults based on Buurtzorg principles: a scoping review protocol.

Objective: The objective of this review is to map the international evidence on the implementation of the Buurtzorg model of community nursing practice for the care of older adults. We will describe where and how it has been used, and the challenges and facilitators of implementing this model of care.

Introduction: The challenges of aging have mobilized health systems around the world to replace the current facility- and disease-centered care model with integrated patient-centered care models.
