A lifelong gluten-free diet (GFD) is the only treatment for celiac disease and other gluten-related disorders. Nevertheless, strict adherence to the GFD is often challenging due to concerns about social isolation, risk of gluten contaminations, high cost, poor quality and the taste of gluten-free products. Moreover, although the GFD is effective in achieving mucosal healing, it may lead to dietary imbalances due to nutrient deficiencies over a long period of time.

Crohn's disease (CD) is a chronic disorder of the digestive tract characterized by an uncontrolled immune-mediated inflammatory response in genetically predisposed individuals exposed to environmental risk factors. Although diet has been identified as one of the major environmental risk factors, the role of nutrients in the clinical management of CD patients has not yet been fully investigated. In this prospective observational study, fifty-four patients diagnosed with active Crohn's disease and undergoing anti-TNF-α biological therapy were enrolled and subjected to nutrient intake analysis through a daily food diary.

Article Synopsis
  • - The management of Helicobacter pylori infection involves a combination of proton pump inhibitors (PPIs) and antibiotics to effectively eradicate the bacteria, as evaluated by the European Registry on Helicobacter pylori management (Hp-EuReg).
  • - Analysis of data from 36,579 patients across five countries revealed that 14-day treatments with specific antibiotic combinations and high-dose PPIs resulted in optimal effectiveness, while 10- and 14-day therapies with high-dose PPIs were also successful.
  • - The study concludes that for first-line treatment, the use of high-dose PPIs is recommended, especially in certain combinations and durations, while shorter 7-day therapies showed suboptimal results.

Inflammatory bowel diseases comprise Crohn's disease and ulcerative colitis, two chronic inflammatory disorders of the digestive tract that develop in adolescence and early adulthood and show a rising pattern in industrialized societies, as well as in developing countries, being strongly influenced by environmental pressures such as nutrition, pollution and lifestyle behaviors. Here, we provide a narrative review of the bidirectional link between nutritional factors and IBD, of dietary deficits observed in IBD patients due to both the disease itself and dietary habits, and of the suggested nutritional interventions. Research of the literature was conducted.

Article Synopsis
  • Primary antibiotic resistance in certain strains is on the rise globally, complicating treatment outcomes, and while tailored therapies based on susceptibility testing have been suggested, their efficacy is still debated.* -
  • A systematic review of 51 studies found that tailored therapies had better success rates (89.9%) compared to empiric therapies (77.6%), especially before first- and second-line treatments, but showed diminished effectiveness before third-line therapies.* -
  • Despite the initial promise of culture-based therapy, a significant number (20%) of patients did not benefi from susceptibility testing, indicating a need for further research to validate the role of bacterial culture in treatment failures.*

: Curing infection remains challenging for clinicians, as no proposed first-line therapy achieves bacterial eradication in all treated patients so that several patients need two or more consecutive treatments. Bacterial culture with antibiotics susceptibility testing is largely unachievable in Italy, and empiric second-line and rescue therapies are generally used. This study aimed to identify what eradication regimens perform better in Italy, following first-line therapy failure.

Cure rate following standard first-line regimens for Helicobacter pylori eradication is decreasing so several patients require two or more treatments. Antibiotic susceptibility-based therapy, advised in current guidelines, is largely impracticable in clinical practice. Some 'standard' regimens (triple therapies based on either levofloxacin or rifabutin, bismuth-based quadruple therapies, sequential, concomitant and hybrid therapies) were empirically used as rescue therapies.
