Solid pseudopapillary pancreatic neoplasm-is a tumor, which appears rarely, predominantly in young women, has low malignant potential and unknown histogenesis. Morphological peculiarities of the tumor may be the same as in endocrinal tumors and ductal pancreatic adenocarcinoma, with which it must be differed. Immunohistochemical investigation of the tumor does not permit to obtain information for strict diferential diagnosis conduction; macroscopic and microscopic peculiarities of the tumor while staining with hematoxilin and eosin give more important information for its diagnosis.

Experience of performance of pancreatic resection during two years in the clinic was summarized. In 90.4% of patients there was revealed malignant tumor, as a rule, in III-IV stage, complicated by jaundice, gastrointestinal bleeding and ileus.

Results of treatment were analyzed in 28 patients with nonresectable pancreatic cancer (III-IV stages) complicated by jaundice, to whom in complex of treatment (biliodigestive shunting, in some of patients with hepatic dearterialyzation, gastroenterostomy, pancreatodigestive shunting) 96% ethanol was injected into the tumor. The dynamics of hemocoagulation system changes in patients with different clinical forms of destructive pancreatitis (DP) was evaluated. A comprehensive evaluation has been done of parameters characterizing cell-bound immunity, cytokinic profile, stress hormones (hydrocortisone), and C-reactive protein in a comparative aspect in those patients having undergone laparoscopic or conventional colectomy. The above indices were determined before surgery, in the early postoperative period, 24 h and 7 days subsequent to surgery. Laparoscopic surgical intervention was found out to lower somewhat the level of immunosuppression early in the postoperative course, which fact is manifested by retaining of expression HLA-DR on monocytes of the peripheral blood.

Overall, thirty-two patients with acute pancreatitis (edematous, destructive, purulent forms) were studied for the state of lipid peroxidation (LPO), antioxidant defence (AOD), and trace element status. The development of destructive and purulent forms of acute pancreatitis was found out to be accompanied by activation of LPO processes, AOD breakdown, with profound disturbances having been disclosed in the metabolism of zinc, selenium, cuprum, manganum, that play an important part in LPO, AOD processes, and in those of immune defence. The secured results suggest that there is a need for us to include into a complex therapy of acute pancreatitis antioxidants and some trace elements (zinc, celenium) and that it is expedient to use indices for the LPO, AOD systems, and for trace element status in the differential diagnosis of clinical forms of acute pancreatitis, prognostication of development of purulent complications.
